    Korelasi Loudness Matching dengan Tinnitus Handicap Inventory (THI) pada Karyawan Pusat Listrik Tenaga Gas (PLTG) Paya Pasir Medan

    Abstract
    Abstract Introduction: Noise is one of the most commo pollutants in the work environment. WHO estimates that approximately 15% of workers in developed country are exposed to noise. Tinnitus is a common accompanying symptom of noise induced hearing loss which can impact the quality of life. Database revealed noise was the primary traumatic causes of tinnitus. Many patients are convinced that the loudness of their tinnitus hinder habituation, whereas they consider a relatively soft signal as easier to tolerate and integrate into normal life. This supported previous research have reported small or moderate correlation between tinnitus loudness and severity of associated distress and handicap. Purpose: To determine correlation between loudness matching and THI on employees who worked at the gas power plant Methods: The study design is descriptive with cross sectional study. This report is obtained 21 employees who meet the inclusion criteria. Data collection was done through interviews, physical examination of the ear with otoscope, hearing assessment and loudness matching tinnitus measurement with pure tone audiometry and tinitus severity with THI questionnaire. Results: Most employees sufferred from tinnitus aged ≥ 50 years as many as 8 employees (38,1%), ear both ears most involved in 16 employees (76.2%), the duration is the most chronic tinnitus as many as 14 employees (66.7%), 18 employees (85.7%) had a high pitch tinnitus, mild hearing loss most is the most common in 9 (42.9%), the highest degree of THI is 1st degrees in 11 employees (52.4%). There is significant relationship between hearing loss and THI (p=0,037). Correlation between loudness matching and THI is r = 0.328 and p = 0.147. Conclusion: There is no the correlation between tinnitus loudness intensity and severity associated tinitus.
     
    Abstrak Pendahuluan: Bising merupakan salah satu polutan yang paling sering ditemukan di lingkungan kerja. WHO memperkirakan sekitar 15% dari seluruh pekerja di negara berkembang berisiko terpapar bising yang berdampak pada gangguan pendengaran dan tinitus. Tinitus merupakan keluhan yang menyertai GPAB yang memberikan dampak negatif terhadap kualitas hidup pekerja. Data menunjukkan bising merupakan penyebab traumatik utama keluhan tinitus. Banyak pasien menyatakan suara tinitus yang kuat mengganggu aktivitas sedangkan suara tinitus yang pelan lebih mudah ditoleransi dan tidak mengganggu aktivitas. Hal ini didukung penelitian terdahulu mendapatkan adanya korelasi yang rendah sampai sedang antara kekuatan suara tinitus dengan keparahan penyakit dan kecacatan yang diakibatkan. Tujuan: Untuk mengetahui korelasi pengukuran loudness matching dengan tingkat keparahan tinitus menggunakan kuesioner THI pada karyawan di PLTG Medan. Metode: Penelitian bersifat deskriptif dengan rancangan studi potong lintang. Didapatkan 21 orang karyawan yang memenuhi kriteria inklusi. Pengumpulan data dilakukan dengan wawancara, pemeriksaan fisik telinga dengan otoskop, pemeriksaan pendengaran dan loudness matching dengan audiometri nada murni dan pemeriksaan tingkat keparahan tinitus dengan kuesioner THI . Hasil Penelitian: Karyawan terbanyak menderita tinitus usia ≥ 50 tahun sebanyak 8 orang (38,1%), keluhan terbanyak terlibat kedua telinga 16 orang (76,2%), durasi terbanyak adalah kronis sebanyak 14 orang (66,7%), 18 orang (85,7%) memiliki tinitus nada tinggi (high pitch), gangguan pendengaran terbanyak tuli ringan sebanyak 9 orang (42,9%), derajat THI terbanyak adalah derajat 1 sebanyak 11 orang (52,4%). Didapati hubungan derajat gangguan pendengaran dan THI (p=0,037). Korelasi antara loudness matching dengan THI adalah r = 0,328 dan p = 0,147. Kesimpulan: Tidak ada korelasi antara kekuatan suara tinitus dengan tingkat keparahan penyakit.
