    Perbandingan Omeprazol dan Ranitidin dalam Pengobatan Dispepsia Fungsional pada Remaja

    Abstract
    Background Functional dyspepsia is caused by heterogeneous pathogenetic factors. There is still need for an evidence-based comparison of clinical management strategies to provide the rationale treatment for adolescent with functional dyspepsia. Proton pump inhibitors (PPI) and H2-receptor antagonists are widely used in this condition. Objective To compare omeprazole and ranitidine for the treatment of functional dyspepsia in adolescents. Methods We conducted a randomized, single blind, controlled trial of adolescents with functional dyspepsia. Participants were randomly assigned to receive omeprazole 20 mg once daily or ranitidine 150 mg twice daily for 2 weeks. Chi-square test was used for evaluating the recurrency (the proportion of patients without symptoms or with marked improvement) after 4 and 8 weeks. Results Of 84 patients (41 girls, 43 boys), 42 patients received omeprazole and placebo and 42 patients received ranitidine. At week 4, the efficacy was 82% in group omeprazole and 79% group ranitidine (P=0.90). But after 8 weeks, 41% group I were symptoms free or had marked improvement, compared to 9.5% in group II (P<0.001). Adverse event was not found in both groups. Conclusion Omeprazole was significantly more effective than ranitidine for the treatment of adolescent with functional dyspepsia.
     
    Latar Belakang Penelitian berdasarkan bukti klinis untuk menentukan tatalaksana yang rasional pada remaja dengan dispepsia fungsional masih sangat diperlukan. Proton Pump Inhibitors (PPI) dan Antagonis Reseptor H-2 (AH2) merupakan obat yang sering dipakai untuk pengobatan dispepsia fungsional. Tujuan Untuk membandingkan omeperazol dan ranitidin dalam pengobatan dispepsia fungsional pada remaja. Metode Uji klinis acak tersamar tunggal dengan membandingkan efektifitas omeperazol 20 mg sekali sehari dengan ranitidin 150 mg dua kali sehari, diberikan selama 2 minggu.. Uji-t independen untuk menilai frekuensi, dan lama atau durasi sakit perut antara kedua kelompok. Uji Chi-square untuk menilai kekambuhan antara kedua kelompok dan dipantau selama 4 dan 8 minggu. Hasil Sebanyak 84 orang (41 perempuan, 43 laki-laki), terdiri dari 42 orang pada kelompok omeprazol dan 42 orang pada kelompok ranitidin. Pada minggu keempat, didapati efektifitas pengobatan pada kelompok 1, 82% dan 79% pada kelompok 2 (p=0,90). Namun setelah pemantauan 8 minggu, 41% pada kelompok 1 tidak didapati gejala dan tanda, sedangkan pada kelompok 2 hanya 9,5% yang mengalami pengurangan gejala dan tanda (p<0,001). Kesimpulan Omeprazol lebih efektif dibandingkan ranitidin dalam mengurangi frekuensi, durasi dan kekambuhan sakit perut yang disebabkan dispepsia fungsional pada remaja.
