Nurses performance appraisal system is an appraisal system that is conducted by hospital nursing management in appraising the nurses performance in the hospitals. The objective of the research was develop a system of performance appraisal of nurses at the District General Hospital of Dr. Pirngadi Medan. The type of research used is action research. The data were gathered from six instruments, they were interviewed guide, the instruction of focus group discussion (FGD), questionnaire of nurse knowledge on performance appraisal, questionnaire of satisfaction on performance appraisal, questionnaire of patient satisfaction on performance appraisal, and the observation sheet. Participants in this study were 8 persons. Data was analyzed qualitatively and quantitatively. Qualitative data were analyzed with content analysis and Quantitative data were analyzed with statistic descriptive test. This study resulted the change of nurse performance appraisal system that consist of the team, workflow, instrument and SOP that refered to performance appraisal concept, the standard of Indonesian National Work Competence (SKKNI) and the Association of Indonesian National Nurses (PPNI) as basic to conceived performance appraisal instrument and application of caring in performance appraisal process and had the effect on disparity of mean in the knowledge of nurses and nurse satisfaction on nurse performance appraisal and the increasing of patients’ satisfaction. The study recommends to the hospital managerial to make a policy and do nurse performance appraisal regularly and systematically to improve and enhance the quality of nursing care. Sistem penilaian kinerja perawat pelaksana adalah suatu sistem penilaian kinerja yang dilakukan oleh manajemen bidang keperawatan untuk menilai kinerja perawat pelaksana di rumah sakit. Penelitian ini bertujuan untuk mengembangkan sistem penilaian kinerja perawat pelaksana di Rumah Sakit Umum Daerah Dr. Pirngadi Medan. Jenis penelitian yang dipakai adalah action research. Instrumen yang digunakan ada enam yaitu panduan wawancara, panduan focus group discussion (FGD), kuisioner pengetahuan perawat, kuisioner kepuasan perawat, kuisioner kepuasan pasien, dan lembar observasi. Partisipan dalam penelitian ini berjumlah delapan orang perawat. Data dianalisa secara kualitatif dan kuantitatif. Data kualitatif dianalisa dengan konten analisis sedangkan data kuantitatif dianalisa dengan uji statistik deskriptif. Penelitian ini menghasilkan suatu perubahan pada sistem penilaian kinerja perawat pelaksana yang terdiri dari tim penilaian kinerja, alur, instrument, SOP yang merujuk kepada konsep penilaian kinerja, SKKNI dan PPNI sebagai dasar penyusunan instrumen penilaian kinerja dan penerapan unsur caring pada proses pelaksanaan penilaian kinerja, selain itu penelitian ini menghasilkan dampak adanya perbedaan rata-rata pada pengetahuan perawat, kepuasan perawat dan meningkatnya kepuasan pasien. Penelitian ini merekomendasikan kepada pihak manajemen rumah sakit agar membuat kebijakan dan melakukan penilaian kinerja perawat secara berkala dan sistematis untuk memperbaiki dan meningkatkan kualitas pelayanan keperawatan.
