    Hubungan kejadian gejala refluks gastroesofageal dengan konstipasi fungsional pada anak

    Abstract
    Background. Constipation and gastroesophageal reflux are commonly found in the childhood. Delayed colonic transit time in children with functional constipation made the children experience gastroesophageal reflux symptoms. Objective. To assess the relation between symptoms of gastroesophageal reflux and functional constipation in children. Methods. A cross sectional study conducted on June 2014 in an orphanage. Eligible samples were 6 until 18 years old children diagnosed with functional constipation on Rome III criteria. Children without functional constipation matched with age and sex of functional constipation children were obtained. Children with and without functional constipation were asked to fill gastroesophageal reflux questionnaire to determine gastroesophageal reflux symptoms. Data were analyzed by chi square analysis. Results. We enrolled 52 children in the study including 26 children with functional constipation (group I) and 26 children without functional constipation (group II). There were 28 boys and 24 girls. Hiccup and belching were two major symptoms noted in group I vs group II, 80.8% vs 57.7% (P=0.211) and 73.1% vs 65.4% (P=0.244). Regurgitation in group I vs group II was 42.3% vs 19.2% (P=0.071). Difficulty in swallowing in group I vs group II was 34.6% vs 11.5% (P=0.048). Conclusion. Difficulty in swallowing as one of the symptoms of gastroesophageal reflux was significantly related with functional constipation, while others were not.
     
    Latar Belakang. Konstipasi dan refluks gastroesofageal merupakan masalah yang sering dijumpai pada masa anak-anak. Keterlambatan waktu transit kolon dapat menyebabkan anak-anak mengalami gejala refluks gastroesofageal. Tujuan. Untuk menilai hubungan antara gejala refluks gastroesofageal dan konstipasi fungsional pada anak. Metode. Studi cross-sectional dilakukan pada bulan Juni 2014 di panti asuhan Alwashliyah Medan. Sampel adalah anak berusia 6 sampai 18 tahun yang memenuhi kriteria Rome III. Anak tanpa konstipasi fungional yang matching dengan usia dan jenis kelamin yang sama dengan anak dengan konstipasi fungsional dipilih sebagai subjek penelitian. Anak dengan dan tanpa konstipasi fungsional diminta untuk mengisi Gastroesophageal Reflux Questionnaire (GERQ) untuk menentukan gejala refluks gastroesofageal. Data dianalisa dengan analisa kai-kuadrat. Hasil. Terdapat 52 anak dalam studi ini termasuk 26 anak dengan konstipasi fungsional (grup I) dan 26 anak tanpa konstipasi fungsional (grup II), dengan 28 anak laki-laki dan 24 anak perempuan. Cegukan dan sendawa adalah dua gejala yang paling sering muncul pada grup I vs grup II, 80,8% vs 57,7% (P=0.211) and 73.1% vs 65.4% (P=0.244). Regurgitasi pada grup I vs grup II sebesar 42.3% vs 19.2% (P= 0.071). Kesulitan menelan pada grup I vs grup II sebesar 34.6% vs 11.5% (P=0.048). Kesimpulan. Kesulitan menelan yang merupakan salah satu gejala dari refluks gastroesofageal berhubungan secara signifikan dengan konstipasi fungsional, sedangkan gejala yang lain tidak berhubungan secara sgignifikan.
