    Perbandingan Kemampuan Fungsional Anak Penderita Hemofilia dengan Anak yang Normal

    Abstract
    Background. Recurrent joint bleedings in hemophilia patients can lead to joint damage and disabilities, causing problems in daily living. Specialized instruments such as the Functional Independence Score in Hemophilia (FISH) can be used to assess the functional abilities of hemophila patients. Objective. To compare the functional abilities of hemophilia patients using the FISH instrument with their normal peers. Method. A cross-sectional study was conducted at the H. Adam Malik Hospital in November till December 2012 on children with hemophilia and their normal peers as control. Each group was assessed using the FISH instrument. We use t-independent test and Spearman correlation test to compare the results of hemophilia patients with their peers. Results. A total of 42 children were enrolled in this study with 21 children in each group. FISH scores in hemophilia children were significantly lower compared to their normal peers in dressing, standing up from chair, squatting, walking and climbing stairs. There was a strong correlation between the level of F VIII and F IX, and the children’s age with the disability in hemophilia children. Conclusion. The functional abilities of hemophilia children were poorer compared to their normal peers.
     
    Latar Belakang. Perdarahan sendi yang berulang pada penderita hemofilia menyebabkan kerusakan dan kecacatan sendi, sehingga mengganggu kemampuan penderita dalam melakukan aktivitas hidup sehari-hari. Penilaian fungsional ini dapat dilakukan dengan menggunakan instrument khusus seperti Functional Independence Score in Hemophilia (FISH) Tujuan. Untuk menilai kemampuan fungsional anak penderita hemofilia dengan menggunakan instrumen FISH dibanding dengan anak yang normal. Metode. Sebuah studi cross sectional dilakukan di RSUP H. Adam Malik Medan pada bulan November dan Desember 2012 terhadap anak penderita hemofilia dan anak yang normal sebagai kontrol. Masing-masing kelompok dinilai kemampuan fungsionalnya dengan menggunakan instrumen FISH. Hasil kedua kelompok dibandingkan dengan menggunakan uji t independen dan uji korelasi Spearman. Hasil. Empat puluh dua orang anak dimasukkan dalam studi ini (kelompok hemofilia n=21, kelompok kontrol n=21). Nilai FISH anak penderita hemofilia berbeda secara signifikan di aktivitas berpakaian, berdiri dari kursi, jongkok, jalan dan naik tangga. Dijumpai korelasi yang kuat antara kadar F VIII atau F IX, dan usia penderita dengan kemampuan fungsional anak penderita hemofilia. Kesimpulan. Kemampuan fungsional anak penderita hemofilia lebih rendah dibanding dengan anak yang normal.
