    Hubungan Kadar Zinc Plasma dengan Gradasi Ulkus Diabetikum pada Penderita DM Tipe II

    Abstract
    Background : Diabetic ulcer is a open wound on the skin caused by chronic complication such as macroangiopathy and microangiopathy. Failure of the wound healing process are the most responsible to the development of diabetic ulcer besides infection, trauma, neuropathy and vascular diseases. Oxidative stress has also been implicated in the development of chronic complications. Some evidences of research is represent that trace element such as zinc has a role in stress oxidative and wound healing process. Objectives : To analyze the relationship plasma zinc level with clssification of diabetic ulcer. Method : This is an analytic cross sectional study involving 50 subject DM type 2 with diabetic ulcer classified 0 – 4, each subject had been examined and graded according to Wagner classification. Blood sample was taken to measure plasma zinc level. The results were analyzed by statistical analysis using Spearman’s correlation test. Result : Significant association between plasma zinc level with classification of diabetic ulcers with p-value = 0,001 coefficient correlation =0,62. Conclusion : There is a significant association between plasma zinc level with classification of diabetic ulcers.
     
    Latar belakang: Ulkus diabetikum adalah luka terbuka pada permukaan kulit yang disebabkan oleh adanya komplikasi kronik berupa mikroangiopati dan makroangiopati. Kegagalan proses penyembuhan luka telah diakui merupakan faktor yang kontribusinya terbesar untuk terjadinya ulkus diabetikum selain daripada iskemia, neuropati, trauma dan infeksi. Stres oksidatif juga mempunyai peranan dalam perkembangan terjadinya komplikasi kronis. Sejumlah penelitian yang dilakukan akhir-akhir ini telah menunjukkan adanya hubungan trace element seperti zinc yang sangat memegang peranan proses penyembuhan luka dan stres oksidatif. Tujuan: Untuk menganalisis hubungan kadar zinc plasma dengan gradasi ulkus diabetikum. Subjek dan metode: Penelitian analitik dengan rancangan cross-sectional yang melibatkan 50 orang penderita DM tipe-2 dengan ulkus diabetikum dalam gradasi ulkus 0 - 4. Setiap subjek penelitian dilakukan pemeriksaan dermatologi kemudian dan ditentukan gradasi ulkus sesuai klasifikasi Wagner. Kemudian darah diambil untuk mengukur kadar zinc plasma. Hasil dianalisis dengan analisis statistik dengan menggunakan uji korelasi Spearman. Hasil: Ditemukan hubungan yang bermakna antara kadar zinc plasma dengan gradasi ulkus diabetikum, dengan p-value = 0,001, nilai korelasi (r) = 0,621. Kesimpulan: Terdapat hubungan bermakna antara kadar zinc plasma dengan gradasi ulkus diabetikum.
