    Pengaruh Pemberian Heparin Selama 7 Hari terhadap Status Hiperkoagulasi Penderita Ulkus Kaki Diabetik

    Abstract
    Background: Diabetic foot and its complications play very significant roles in mortality and morbidity for diabetic patients. Thrombosis influences the impact in diabetic foot management. In general, diabetes will be followed by prothrombotic state that is changes in thrombolysis and fibrinolysis processes. Thrombosis is one of the complication factors to increase the number of morbidity and mortality in managing the diabetic foot. One of the methods in order to lower the chance of handicapped and death due to diabetic foot is giving anticoagulant and antiplatelet to the patients. Until now the use of anticoagulant has not become the main interest in dealing with diabetic foot ulcer. Heparin works through an indirect effects of thrombolysis and anticoagulation. Therefore we would like to investigate the efffectiveness of heparin in hypercoagulation management of the diabetic foot. Objective: To assess the change of hypercoagulation status in patients with diabetic foot ulcer after Heparin treatment compared with control. Method: We conducted clinical trial on 16 subjects of diabetic foot ulcer patients with hipercoagulation status and measured coagulation status parameter after oral Heparin 5000 U three times a day for 7 days and compared with 16 controls whom did not give the heparin therapy. Results: After 7 days therapy, the resuls shown that there was a significant increase in mean of D dimer (p=0,05) and decrease in fibrinogen (p=0,045). While there were no significant changes in mean of PT ratio, INR, aPTT ratio, and TT ratio in control patients. Conclusion: 7 days subcutaneous heparin therapy for diabetic foot ulcer patients is very important and useful, which can be seen by the increase of mean of D dimer that indicated the fibrinolytic activity was enhanced. Therefore we found decreased of fibrinogen that indicated improvement in hypercoagulation.
     
    Latar belakang: Ulkus kaki dan komplikasinya merupakan penyebab penting mortalitas dan morbiditas penderita diabetes. Secara umum Diabetes Mellitus akan disertai dengan keadaan protrombotik yaitu perubahan-perubahan proses trombosis dan fibrinolisis. Trombosis menjadi salah satu penyulit yang dapat meningkatkan angka morbiditas dan mortalitas dalam pengelolaan komplikasi ulkus kaki diabetik. Salah satu upaya untuk mengurangi kecacatan dan kematian akibat ulkus kaki diabetik dapat dilakukan dengan mencegah terjadinya trombosis melalui pemberian antikoagulan dan anti aggregasi trombosit. Sampai saat ini penggunaan antikoagulan belum menjadi perhatian dalam penanganan ulkus kaki diabetik. Heparin dikatakan bekerja melalui efek trombolisis secara tidak langsung dan efek antikoagulasi. Untuk itu kami ingin mengetahui efektifitas dari Heparin dalam penanganan hiperkoagulasi pada ulkus kaki diabetik. Tujuan: Untuk mengetahui adanya perubahan status koagulasi pada penderita ulkus kaki diabetik setelah pemberian heparin dibandingkan dengan kontrol. Metode: 16 subjek penderita ulkus kaki diabetik dengan status hiperkoagulasi dilakukan uji klinis dengan mengukur parameter status koagulasi sebelum dan sesudah pemberian Heparin 5000 3 kali sehari selama 7 hari dan dibandingkan dengan 16 orang kontrol yang tidak mendapat Heparin. Hasil: Setelah 7 hari terapi didapatkan peningkatan rerata D dimer yang signifikan (p=0,05) dan penurunan fibrinogen pada subjek (p=0,045) dibandingkan kontrol sementara rerata rasio PT,INR, rasio aPTT, rasio TT tidak dijumpai perubahan yang bermakna. Kesimpulan: Pemberian Heparin selama 7 hari pada penderita ulkus kaki diabetik dengan status hiperkoagulasi bermakna ditandai oleh peningkatan kadar D dimer sebagai penanda peningkatan aktifitas fibrinolisis dan perbaikan koagulasi yang ditandai penurunan kadar fibrinogen menuju normal.
