dc.description.abstract | Background : Stroke is an abnormality of brain function caused by distrubtion of the circulation to brain. Several studies stated that adiponectin and triglyceride serum associated with infarct volume and post stroke outcomes, so it is useful to predict the brain damage and outcome. Our objectives was to study relationship between serum adiponectin and triglyceride with infarct volume and outcome in acute ischemic stroke. Methods : This was an observational study of 32 acute ischemic stroke patients admitted to Neurological ward at School of Medicine, University of Sumatera Utara / H.Adam Malik Hospital Medan, from February 2008 to October 2008. Diagnosis of stroke was established on history, physical and neurological examinations and neuroimaging. Blood vein was drawn for adiponectin and triglyceride in 72 hours since admission. Outcome was measured with National Institute of Health Stroke Scale (NIHSS), Barthel Index (BI) and Modified Rankin Scale (mRS) on first, seventh and fourteenth days. Results : The subjects were consisted of 17 male (53,1%) and 15 female (46,9%), mean age was 62,16 years. The mean of adiponectin and triglyceride level were 6,07±11,51 ?g/mL and 120,53±46,93 mg/dL. There were negative correlation between adiponectin level with NIHSS score in first day (r=-0,407 p=0.021), seventh day (r=-0,390 p=0,027), fourteenth day (r=-0,270 p=0,135) and mRS score in first day (r=-0,447 p=0,010), seventh day (r=-0,400 p=0,023), fourteenth day (r=-0,393 p=0,026) ; and positive correlation with BI score in first day (r=0.504 p=0,003), seventh day (r=0,467 p=0,007) and fourteenth day (r=0,452 p=0,009). There were negative correlation between triglyceride level with : NIHSS score in first day (r=-0,137 p=0.455), seventh day (r=-0,115 p=0,532), fourteenth day (r=-0,079 p=0,667) and mRS score in first day (r=-0,182 p=0,319), seventh day (r=-0,127 p=0,488) and fourteenth day (r=-0,103 p=0,577); and positive correlation with BI score in first day (r=0.504 p=0,003), seventh day (r=0,467 p=0,007) and fourteenth day (r=0,452 p=0,009). Infarct volume had negative correlation with adiponectin level (r=-0,139 p=0,449) and triglyceride (r=-0,340 p=0,057). Conclusion : Increased in serum adiponectin level correlated significantly with: decreased NIHSS and mRS scores; and increased BI score. Increased in serum triglyceride level correlated with: decreased NIHSS and mRS scores; and increased BI score, but not significant. Increased in adiponectin and triglyceride level correlated with decreased infarct volume but not significant. | en_US |
dc.description.abstract | Latar belakang : Stroke merupakan suatu abnormalitas fungsi otak akibat terputusnya sirkulasi ke otak. Beberapa studi menyatakan adiponektin dan trigliserida serum berhubungan dengan volume infark dan outcome pasca stroke, sehingga berguna untuk memprediksi kerusakan otak dan outcome yang terjadi. Tujuan penelitian ini adalah untuk mengetahui hubungan kadar adiponektin dan trigliserida serum dengan volume infark dan outcome pada pasien stroke iskemik akut. Metode : Penelitian ini merupakan studi observasional terhadap 32 pasien stroke iskemik akut yang dirawat di Bangsal Neurologi FK-USU/RSUP.H.Adam Malik Medan periode Pebruari 2008 hingga Oktober 2008. Diagnosis stroke dilakukan dengan anamnesis, pemeriksaan klinis dan neurologik serta neuroimejing. Pengambilan darah vena untuk pemeriksaan kadar adiponektin dan trigliserida dilakukan dalam 72 jam setelah pasien masuk rumah sakit. Pengukuran outcome dilakukan dengan National Institute of Health Stroke Scale (NIHSS), Barthel Index (BI) dan Modified Rankin Scale (mRS) pada hari pertama, ketujuh dan keempat belas. Hasil : Subjek terdiri dari 17 laki-laki (53,1%) dan 15 perempuan (46,9%) dengan rerata umur 62,16 tahun. Nilai rerata adiponektin dan trigliserida masing-masing 6,07±11,51 ìg/mL dan 120,53±46,93 mg/dL. Terdapat korelasi negatif antara kadar adiponektin dengan : skor NIHSS hari pertama (r=-0,407 p=0.021), hari ketujuh (r=-0,390 p=0,027), hari keempat belas (r=-0,270 p=0,135) dan mRS hari pertama (r=-0,447 p=0,010), hari ketujuh (r=-0,400 p=0,023), hari keempat belas (r=-0,393 p=0,026) ; serta berkorelasi positif dengan skor BI hari pertama (r=0.504 p=0,003), hari ketujuh (r=0,467 p=0,007) dan hari keempat belas (r=0,452 p=0,009). Peningkatan kadar trigliserida serum berhubungan dengan : penurunan skor NIHSS dan mRS; dan peningkatan skor BI, namun tidak bermakna. Peningkatan kadar adiponektin dan trigliserida serum berhubungan dengan penurunan volume infark, namun tidak signifikan. | en_US |
