    Uji Diagnostik Toronto Clinical Scoring System Terhadap Semmes-Weinstein Monofilament Test pada Neuropati Kusta

    Abstract
    Background: Leprosy is a chronic disease caused by Mycobacterium leprae which can cause damage to peripheral nerves. In general, if damage to nerve function is not treated quickly and appropriately, disability will occur, so early detection of damage to peripheral nerves is very necessary. Semmes-Weinstein Monofilament Test is an examination that has been widely used to assess peripheral neuropathy in leprosy but is not practical in the field. The Toronto Clinical Scoring System (TCSS) is a simple neuropathy scoring system and has proven superiority in diabetic neuropathy, chemotherapy-induced peripheral neuropathy and human immunodeficiency virus. Toronto Clinical Scoring System is expected to be an alternative in diagnosing neuropathy in leprosy patients in a simpler, faster, and with a good level of accuracy. Objective: To determine the diagnostic value of TCSS on the SWM test in leprosy neuropathy. Subjects and methods: This study is an observational analytic study with a cross-sectional design involving 40 leprosy patients. In the research subjects, basic data was collected, neuropathy examination using TCSS scores and Semmes-Weinstein Monofilament scores. The collected data was then processed and statistical analysis was carried out to assess the sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio, accuracy, and AUC value of this TCSS examination. Results: Of the 40 research subjects, there were 21 people diagnosed with leprosy neuropathy using the SWM test and with the TCSS score. From the analysis of the diagnostic test for the TCSS score, the sensitivity was 85.7%, specificity was 84.2%, PPV was 85.7%, NPV was 84.2%, LR+ was 5.42, LR- was 0.17, accuracy was by 85%, and the AUC value of 85%. Conclusion: TCSS can be used as a tool to diagnose neuropathy in leprosy patients.
     
    Latar belakang: Kusta merupakan penyakit kronis yang disebabkan oleh Mycobacterium leprae dapat menimbulkan kerusakan pada saraf perifer. Secara umum, jika kerusakan fungsi saraf tidak ditangani dengan cepat dan tepat, akan terjadi kecacatan, sehingga deteksi dini kerusakan pada saraf perifer sangat diperlukan. Semmes-Weinstein Monofilament Test merupakan pemeriksaan yang telah digunakan secara luas untuk menilai neuropati perifer pada kusta namun kurang praktis dikerjakan di lapangan. Toronto Clinical Scoring System (TCSS) merupakan suatu sistem penilaian neuropati yang sederhana dan telah terbukti keunggulannya pada neuropati diabetes melitus, neuropati perifer terinduksi kemoterapi dan Human immunodeficiency virus. Toronto Clinical Scoring System diharapkan dapat menjadi suatu alternatif dalam mendiagnosis neuropati pada pasien kusta dengan cara yang lebih sederhana, cepat, dan dengan tingkat akurasi yang baik. Tujuan: Mengetahui nilai diagnostik TCSS terhadap SWM test pada neuropati kusta. Subjek dan metode: Penelitian ini merupakan suatu studi analitik observasional dengan rancangan potong lintang yang melibatkan 40 pasien kusta. Pada subjek penelitian dilakukan pengambilan data dasar, pemeriksaan neuropati menggunakan skor TCSS dan Semmes-Weinstein Monofilament. Data yang terkumpul kemudian diolah dan dilakukan analisis statistik untuk menilai sensitivitas, spesifisitas, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio, akurasi, serta nilai AUC dari pemeriksaan TCSS ini. Hasil: Dari 40 subjek penelitian, terdapat 21 orang yang didiagnosis mengalami neuropati kusta dengan menggunakan SWM test dan dengan skor TCSS. Dari analisa uji diagnostik untuk skor TCSS didapatkan sensitivitas sebesar 85,7%, spesifisitas sebesar 84,2%, PPV sebesar 85,7%, NPV sebesar 84,2%, LR+ sebesar 5,42, LR- sebesar 0,17, akurasi sebesar 85%, dan nilai AUC sebesar 85%. Kesimpulan: TCSS dapat digunakan sebagai alat untuk mendiagnosis neuropati pada pasien kusta.
