    Identifikasi Sarcoptes Scabiei dengan Pemeriksaan Klinis dan Pemeriksaan Penunjang di Kota Medan

    Abstract
    Background: Scabies is an infectious disease caused by Sarcoptes scabiei var. hominis. The diagnosis of scabies is made by pathognomonic clinical symptoms of scabies, but sometimes the clinical symptoms are not typical due to similarities with other skin diseases. It is necessary to establish a definitive diagnosis to find Sarcoptes scabiei or its eggs with supporting examinations, such as microscopic, dermoscopy, and polymerase chain reaction (PCR). Objective: to evaluate the identification of Sarcoptes scabiei between clinical symptoms and supporting examinations. Methods: Descriptive cross-sectional study, simple random sampling with a sample of 50 people who met the inclusion and exclusion criteria, then a physical examination was carried out to determine the pathognomonic scabies lesion, then a dermoscopy was performed, after which skin scrapings were performed for microscopic examination and PCR. The results of the examination are described in tabular form. Results: In 50 samples, identification of a typical scabies rash based on location showed on head 2 people (4%), wrists 7 (14%), fingers 13 people (26%), toes 13 people (26%), groin as many as 2 people (4%), buttocks 7 people (14%), and external genitalia as many as 6 people (12%). On supporting examination, the results of microscopic examination as the gold standard did not reveal any scabies (0.0%), with other supporting examinations, such as dermoscopy, no scabies (0.0%) and PCR found 12 positive samples of scabies (24%). Conclusion: Sarcoptes scabiei can be detected through PCR examination even with a very small number of skin scrapings.
     
    Latar belakang: Skabies adalah penyakit menular yang disebabkan oleh tungau Sarcoptes scabiei var. hominis. Diagnosis skabies ditegakkan dengan gejala klinis patognomonis skabies, namun terkadang gejala klinis tersebut tidak khas akibat adanya kemiripan dengan penyakit kulit lainnya. Diperlukan penegakan diagnosis yang definitif untuk menemukan Sarcoptes scabiei ataupun telurnya dengan pemeriksaan penunjang yaitu mikroskopis, dermoskopi, dan polymerase chain reaction (PCR). Tujuan: mengevaluasi identifikasi Sarcoptes scabiei antara gejala klinis dengan pemeriksaan penunjang. Metode: Studi cross-sectional bersifat deskriptif, simple random sampling dengan jumlah sampel 50 orang yang memenuhi kriteria inklusi dan eksklusi kemudian dilakukan pemeriksaan fisik untuk menentukan lesi patognomonis skabies, selanjutnya dilakukan pemeriksaan dermoskopi, setelah itu dilakukan kerokan kulit untuk pemeriksaan mikroskopi dan PCR. Hasil pemeriksaan dideskripsikan dalam bentuk tabel. Hasil: pada 50 sampel dijumpai hasil identifikasi ruam khas skabies berdasarkan lokasi yang menunjukkan di kepala sebanyak 2 orang (4%), pergelangan tangan sebanyak 7 orang (14%), sela jari tangan 13 orang (26%), sela jari kaki 13 orang (26%), selangkangan sebanyak 2 orang (4%), bokong 7 orang (14%), dan genitalia eksterna sebanyak 6 orang (12%). Pada pemeriksaan penunjang tampak hasil pemeriksaan mikroskopis sebagai gold standard tidak ditemukan adanya skabies (0,0%), dengan pemeriksaan penunjang lainnya yaitu dermoskopi tidak ditemukan skabies (0,0%) dan PCR dijumpai 12 sampel positif skabies (24%). Simpulan: Sarcoptes scabiei dapat dideteksi melalui pemeriksaan PCR walaupun dengan dengan jumlah kerokan kulit yang sangat sedikit.
