Hubungan Kadar Troponin I dengan Skor Grace Pada Pasien Sindrom Koroner Akut di RSUP H. Adam Malik Medan

Abstract

Background: Acute coronary syndrome (ACS) describes a wide range of myocardial ischemia, which is always associated with the rupture of atherosclerotic plaques in coronary arteries. CRP has been shown to be a risk predictor of ACS. A study showed that a 0.4% mortality rate in patients with normal CRP had increased to 5.8% in patients with elevated CRP levels. GRACE score has been associated with predicting early and late death in ACS patients, although the association between hs-CRP levels and GRACE score is still not known. Objective: To discover the association between hs-CRP levels with GRACE score in ACS patients in H. Adam Malik Medan Central General Hospital. Methods: This study was a retrospective analytic study. The study population was acute coronary syndrome patients in RSUP (central general hospital) H. Adam Malik Medan, and 51 subjects met the inclusion and exclusion criteria. Data analysis was done using univariate and bivariate analysis. A correlation test was carried out using the Spearman correlation test to find the association between hs-CRP levels with GRACE score in ACS patients. Results: There were 51 samples included in the study. There was a significant difference (p = 0.005) in the mean hs-CRP levels in UAP, NSTEMI, and STEMI subjects. Conversely, there was no significant difference in GRACE score among the three ACS diagnoses. For the association of hs-CRP levels with GRACE score in all subjects, the Spearman correlation test showed a significant correlation between hs-CRP levels with GRACE score (p < 0.001) with a correlation coefficient (r) of 0.588. Conclusion: From our results, it can be concluded that there is a significant association between serum hs-CRP levels with GRACE score; and the higher hs-CRP levels will be followed by a higher GRACE score also.

Latar Belakang: Sindrom koroner akut (SKA) menggambarkan berbagai rentang iskemik miokard yang selalu dikaitkan dengan rupturnya plak aterosklerosis di arteri koroner. CRP telah terbukti sebagai prediktor resiko SKA. Dari suatu studi diketahui bahwa tingkat kematian 0,4% pada pasien dengan CRP normal naik dari 0,4% menjadi 5,8% ketika kadar CRP juga meningkat. Skor GRACE dikaitkan dengan prediksi kematian dini dan lanjut pada pasien SKA, tetapi keterkaitan dari kadar hs-CRP dengan skor GRACE masih belum banyak diketahui Tujuan: Mengetahui hubungan hubungan kadar hs-CRP dengan skor GRACE pada pasien SKA di RSUP H. Adam Malik Medan. Metode: Penelitian ini merupakan penelitian analitik retrospektif. Populasi penelitian adalah penderita sindrom koroner akut yang dirawat di RSUP H. Adam Malik Medan dan didapatkan subyek penelitian sebesar 51 orang yang memenuhi kriteria inklusi dan eksklusi. Analisis data dilakukan menggunakan analisis univariat dan bivariat. Uji korelasi dilakukan menggunakan uji korelasi Spearman untuk melihat hubungan kadar hs-CRP dengan skor GRACE pada penderita SKA Hasil: Terdapat 51 sampel yang mengikuti penelitian ini. Didapatkan perbedaan bermakna (p = 0.005) rerata kadar hs-CRP pada subyek dengan UAP, NSTEMI, dan STEMI. Sedangkan skor GRACE tidak didapatkan perbedaan bermakna diantara ketiga diagnosis SKA. Untuk hubungan kadar hs-CRP dengan skor GRACE untuk seluruh subyek, uji korelasi Spearman menunjukkan bahwa terdapat korelasi yang signifikan antara kadar Hs-CRP dengan skor GRACE (p < 0,001) dengan nilai korelasi (r) adalah sebesar 0,588. Kesimpulan: Dari hasil penelitian ini, maka dapat disimpulkan bahwa terdapat hubungan yang bermakna antara kadar hs-CRP serum dengan skor GRACE dan semakin tinggi kadar hs-CRP maka akan diikuti oleh peningkatan skor GRACE
