Perbedaan Mean Platelet Volume dan Luaran Klinis setelah Pemberian Dual Antiplatelet atau Monoterapi Aspirin dengan atau tanpa Statin pada Pasien Stroke Iskemik Akut

Abstract

Background Antiplatelet therapy is an important therapy in the management of ischemic stroke. Many studies have shown that dual antiplatelet therapy is more effective than aspirin monotherapy in preventing vascular events in subjects with acute phase minor stroke. In addition to antiplatelet agents, statin therapy has been shown to be important in stroke prevention. Positive results from clinical trials have evaluated the efficacy and safety of statins in stroke, leading to increased use of statins for stroke patients. Objective This study was conducted to determine the difference in mean platelet volume and clinical outcome after dual antiplatelet or aspirin monotherapy with or without statins. Method This study used a quasi-experimental design in patients with acute ischemic stroke. The subjects who participated in the study were 36 acute ischemic stroke patients who were divided into four groups, namely groups I, II, III and IV with 9 people in each group. Group I received dual antiplatelet and statin therapy, group II received dual antiplatelet therapy, group III received aspirin and statin monotherapy and group IV received aspirin monotherapy. Result There were differences in MPV levels after the seventh day in the group of patients given dual antiplatelet therapy with statins (p= 0.007). There was no difference in NIHSS scores after the seventh day in the four intervention groups (p > 0.05). There was no difference in the Barthel Index value after the seventh day in the four intervention groups (p> 0.05). There was a delta difference in MPV levels between the four intervention groups (p= 0.020). There was no difference in the NIHSS delta scores between the four intervention groups (p = 0.167). There was no difference in the delta value of the Barthel Index between the four intervention groups (p = 0.255) Conclusion There was a delta difference in MPV levels between the four intervention groups. There was no difference in the NIHSS and Barthel Index delta scores between the four intervention groups.

Latar Belakang Pemberian antiplatelet merupakan terapi penting dalam manajemen stroke iskemik. Saat ini banyak studi yang menyatakan bahwa pemberian terapi dual antiplatelet lebih efektif daripada monoterapi aspirin dalam mencegah kejadian vaskular pada subjek dengan stroke minor fase akut. Selain antiplatelet, terapi statin telah diketahui penting dalam pencegahan stroke. Hasil positif dari uji klinis telah mengevaluasi kemanjuran dan keamanan statin pada stroke, yang mengarah pada peningkatan penggunaan statin untuk pasien stroke. Tujuan Penelitian ini dilakukan untuk mengetahui perbedaan mean platelet volume dan luaran klinis setelah pemberian dual antiplatelet atau monoterapi aspirin dengan atau tanpa statin. Metode Penelitian ini menggunakan design quasi experimental pada penderita stroke iskemik akut. Subjek yang mengikuti penelitian sebanyak 36 pasien stroke iskemik akut yang dibagi menjadi empat kelompok yaitu kelompok I, II, III dan IV dengan masing-masing kelompok berjumlah 9 orang. Kelompok I memperoleh terapi dual antiplatelet dan statin, kelompok II mendapat terapi dual antiplatelet, kelompok III mendapat terapi monoterapi aspirin dan statin dan kelompok IV mendapat monoterapi aspirin. Hasil Terdapat perbedaan kadar MPV setelah hari ketujuh pada kelompok pasien yang diberikan terapi dual antiplatelet dengan statin (p= 0,007). Tidak terdapat perbedaan skor NIHSS setelah hari ketujuh pada keempat kelompok intervensi (p> 0,05). Tidak terdapat perbedaan nilai Barthel Index setelah hari ketujuh pada keempat kelompok intervensi (p> 0,05). Terdapat perbedaan delta kadar MPV antara keempat kelompok intervensi (p= 0,020). Tidak terdapat perbedaan delta skor NIHSS antara keempat kelompok intervensi (p= 0,167). Tidak terdapat perbedaan delta nilai Barthel Index antara keempat kelompok intervensi (p= 0,255) Kesimpulan Terdapat perbedaan delta kadar MPV antara keempat kelompok intervensi.
