    Hubungan Polimorfisme GSTP1 terhadap Kejadian Resistensi Imatinib Mesilat Pada Pasien CML di RSUP HAM Beserta Rumah Sakit Jejaring di Kota Medan

    Abstract
    Variations in the genes coding for drug metabolism enzymes may explain the variability response to treatment. Previous studies reported that GSTP1 polymorphism was associated with the incidence of Imatinib Mesylate resistance as first-line therapy in CML patients. This study aims to determine the association between GSTP1 polymorphism and Imatinib Mesylate resistance in CML patients. Methods : A total of 46 people consisting of 23 CML patients with Imatinib Mesilat resistance and 23 CML patients without Imatinib Mesilat resistance at H. Adam Malik Hospital and Branch Hospital in Medan City, Indonesia. Blood samples were taken for examination of GSTP1 polymorphism through Polymerase Chain Reaction (PCR). Resistance status was taken from medical records based on ELN criteria. Data were analyzed using Fisher's Exact test. A p value less than 0.05 was applied to each statistical test as significant. Result and Discussion : The distribution of research subjects characteristic was divided into imatinib resistance and non-Imatinib resistance groups. In the imatinib resistance group, the highest frequency was found in Javanese and GSTP1polymorphism Ile/Ile. In the non-imatinib-resistant group, the highest frequency was found in Batak tribe and GSTP1 polymorphism Ile/Val. There was no significant relationship between the GSTP1 polymorphism and the incidence of resistance in CML patients (p>0.05). This can be attributed to geographical differences and ethnic differences in some studies. Conclusion: There was no relationship between GSTP1 polymorphism and the incidence of resistance to imatinib mesylate in CML patients.
     
    Latar Belakang & Tujuan : Variasi pada gen – gen yang mengkode enzim yang berperan dalam metabolisme obat dapat menjelaskan adanya variabilitas respons terhadap pengobatan. Penelitian terdahulu melaporkan bahwa Polimorfisme GSTP1 memiliki hubungan terhadap kejadian resistensi Imatinib Mesilat sebagai lini pertama terapi pada pasien CML. Penelitian ini bertujuan untuk mengetahui hubungan antara polimorfisme GSTP1 terhadap kejadian resistensi Imatinib Mesilat pada pasien CML. Metode: Subjek penelitian sebanyak 46 orang (23 pasien CML dengan resistensi Imatinib Mesilat dan 23 pasien CML tanpa resistensi Imatinib Mesilat) di RSUP H. Adam Malik dan Rumah Sakit Jejaring di Kota Medan. Sampel darah diambil untuk pemeriksaan polimorfisme GSTP1 melalui Polymerase Chain Reaction (PCR). Status Resistensi diambil dari rekam medik berdasarkan Kriteria ELN. Data dianalisis menggunakan uji Fisher Exact. Nilai p < 0,05 dianggap bermakna secara statistik. Hasil dan Pembahasan: Distribusi karakteristik subjek penelitian dibagi menjadi kelompok resistensi imatinib dan tidak resistensi Imatinib. Pada kelompok resistensi imatinib, frekuensi tertinggi berada pada suku jawa dan polimorfisme GSTP1 Ile/Ile. Pada kelompok tidak resistensi imatinib, frekuensi tertinggi berada pada suku batak dan polimorfisme GSTP1 Ile/Val. Tidak dijumpai adanya hubungan yang signifikan antara polimorfisme GSTP1 terhadap kejadian resistensi pada pasien CML (p>0,05). Hal ini dapat dikaitkan dengan perbedaan geografis dan perbedaan etnis pada beberapa penelitian. Kesimpulan: Tidak dijumpai adanya hubungan antara polimorfisme GSTP1 terhadap kejadian resistensi imatinib mesilat pada penderita CML.
