dc.description.abstractBackground: Neurological manifestations in Covid-19 patients need special attention because of high morbidity and mortality, the mortality rate of Covid-19 patients with neurological manifestations is high, which is around 38%. Because of the high mortality rate due to Covid-19, it is necessary to develop a clinical score for determine the probability of mortality in Covid-19 patients during hospitalization and this clinical score can be used as a guideline for a clinician to provide comprehensive management to reduce mortality rates, especially during the hospitalization. Aim: This study aimed to develop and validate the Hospital Mortality predictive model in Covid-19 patients with neurological manifestations. Methods: This study was a retrospective cohort study in Covid-19 patients with neurological manifestations at H. Adam Malik Hospital from April 2020 to March 2022. Score development was done by collecting and processing variables that theoretically affected hospital mortality in Covid-19 patients with neurological manifestations. Score calibration was assessed by the Hosmer-Lemeshow good-of-fit test and the statistical discrimination by calculating the area under the curve (AUC) on the Receiver Operating Characteristic (ROC) curve. Results: From 406 Covid-19 patients with neurological manifestations were included in the analysis (mean age: 53.90 ± 14.93, 53.7% male). There were 7 score-forming variables, age, comorbidities, thoracic radiological findings, 3 laboratory parameters, namely D-Dimer, NLR, and CRP values as well as neurological clinical manifestations of the central nervous system with a maximum total score of 12. Predictor scores showed good calibration (Hosmer-Lemeshow p= .997) and strong statistical discrimination (AUC .994, 95% CI: .990-.999). Conclusion: Hospital mortality predictive model has been developed in Covid-19 patients with neurological manifestations with 7 score-forming components that have good validity.en_US
dc.description.abstractLatar Belakang: Kelainan neurologis pada pasien Covid-19 sangat memerlukan perhatian khusus karena morbiditas dan mortalitasnya yang tinggi, angka mortalitas pasien Covid-19 dengan kelainan neurologis cukup tinggi yaitu sekitar 38% .Mengingat tinggi nya angka kematian akibat Covid-19 maka perlu dikembangkan suatu skor klinis untuk mengetahui probabilitas mortalitas pasien Covid-19 selama rawatan. Diharapkan dengan adanya skor klinis tersebut dapat menjadi pedoman seorang klinisi untuk memberikan tatalaksana komprehensif guna mengurangi tingkat mortalitas khususnya saat dalam rawatan. Tujuan : Penelitian ini bertujuan untuk melakukan pengembangan dan validasi skor prediktor Hospital Mortality pada pasien Covid-19 dengan manifestasi klinis neurologis. Metoda: Penelitian ini merupakan studi kohort retrospektif pada pasien Covid-19 dengan manifestasi klinis neurologis di RSUP H. Adam Malik periode April 2020 sampai Maret 2022. Pengembangan skor dengan mengumpulkan dan mengolah variabel variabel yang secara teori mempengaruhi hospital mortality pada pasien Covid-19 dengan manifestasi klinis neurologis. Kalibrasi skor dinilai dengan uji good-of-fit Hosmer-Lemeshow dan kekuatan diskriminasinya dengan menghitung Area Under the curve (AUC) pada kurva Receiver Operating Characteristic (ROC). Hasil: Secara total ada, 406 pasien Covid-19 dengan manifestasi klinis neurologis dimasukkan dalam analisis (Usia rata-rata: 53,90 ± 14,93 dan 53,7 % laki-laki). Didapatkan 7 variabel pembentuk skor prediktor yaitu usia, komorbid, temuan radiologi toraks, 3 parameter laboratorium yaitu nilai D-Dimer, NLR, dan CRP serta manifestasi klinis neurologis sistem saraf pusat dengan total skor maksimal 12. Skor prediktor menunjukkan kalibrasi yang baik (Hosmer-Lemeshow p= .997) dan kekuatan diskriminasi yang sangat kuat (AUC .994, 95% CI: .990-.999). Kesimpulan: Telah dikembangkan skor prediktor Hospital mortality pada pasien Covid-19 dengan manifestasi neurologis dengan 7 komponen pembentuk skor yang memiliki validitas yang baik.en_US
