    Hubungan Polimorfisme Gen Reseptor Vitamin D Fok I dengan Periodontitis Kronis

    Abstract
    Background: The vitamin D receptor (VDR) plays an important role in bone metabolism and immune response modulation, both of which are important for the development of chronic periodontitis (CP). Therefore the polymorphism of VDR could give significant effect on the CP susceptibility. Aim: This study investigated the association between the VDR polymorphism FokI and chronic periodontitis. Method: This is a case-control study involving 43 CP patients and 40 periodontally healthy control subjects. The DNA was extracted from the buccal epithelial cell and the VDR FokI polymorphism were determined by using the PCR-RFLP (Polymerase Chain Reaction-Restriction Fragment Length Polymorphism). The frequencies of genotypes and alleles were compared between patients and control subjects using chi-square and odd ratio measured by chi-square and regression logistic. Result: The genotype frequencies of VDR Fok I are 44,2% FF, 48,8% Ff and 7% ff in chronic periodontitis group, while 25% FF, 50% Ff and 25% ff in control group. The frequency of genotypes was in Hardy-Weinberg equilibrium with p value > 0,05. Significant association are found at genotypes FF compare to ff (p=0,016), while the genotype Ff did not show any significant association compare to ff genotype (p>0,05). The FF genotypes had the potential to increase the susceptibility of chronic periodontitis compare to ff genotypes with OR 6,33 (95% CI: 1,413-28,39), so did the (FF + Ff) genotypes compare to ff genotypes with OR 4,44 (95% CI: 1,12-17,56) as well as the F allele compare to f allele with OR 2,18 (95% CI: 1,16-4-11). Conclusion: The current findings confirm that the polymorphism of VDR FokI were significantly associated with chronic periodontitis.
     
    Latar Belakang: Reseptor Vitamin D (RVD) memiliki peran dalan metabolisme tulang dan modulasi respon imun dimana keduanya penting dalam patogenesis periodontitis kronis. Oleh karena itu, polimorfisme pada RVD dapat mempengaruhi kerentanan terjadinya periodontitis kronis. Tujuan: untuk mengetahui hubungan polimorfisme gen reseptor vitamin D Fok I terhadap kerentanan terjadinya Periodontitis Kronis Metode: Desain penelitian adalah case-control dengan 43 pasien kasus periodontitis kronis dan 40 subjek kontrol dengan periodontal sehat. DNA diekstraksi dari sampel sel epitel bukal dan polimorfisme gen RVD Fok I diperiksa dengan teknik PCR-RFLP (Polymerase Chain Reaction-Restriction Fragment Length Polymorphism). Perbedaan proporsi genotip dan alel antara kedua kelompok diuji dengan chi square sementara odd ratio diukur dengan menggunakan uji chi-square dan regresi logistik. Hasil: Frekuensi genotip RVD FokI adalah 44,2% FF, 48,8% Ff dan 7% ff pada kelompok kasus periodontitis kronis serta 25% FF, 50% Ff dan 25% ff pada kelompok kontrol. Terdapat kesesuaian genotip gen reseptor vitamin D Fok I dengan keseimbangan Hardy-Weinberg dengan nilai p>0,05. Hubungan signifikan terjadinya periodontitis kronis ditemukan pada subjek dengan genotip FF dibandingkan genotip ff (p=0,016). Sementara antara genotip Ff dibandingkan genotip ff tidak ditemukan hubungan signifikan (p>0,05). Genotip FF gen reseptor vitamin D Fok I terbukti meningkatkan risiko periodontitis kronis dibandingkan genotip ff dengan nilai OR 6,33 (95% CI: 1,413-28,39). Genotip (FF + Ff) terbukti meningkatkan risiko periodontitis kronis dibandingan subjek dengan genotip ff dengan OR 4,44 (95% CI: 1,12-17,56). Alel F gen reseptor vitamin D Fok I terbukti meningkatkan risiko periodontitis kronis dengan OR 2,18 (95% CI: 1,16-4-11). Kesimpulan: Pada penelitian ini ditemukan hubungan bermakna polimorfisme gen reseptor vitamin D Fok I dengan periodontitis kronis.
