dc.description.abstract | Background: Obesity and hyperlipidemia could effect the development and progessivity of the microvascular complication such as diabetic neuropathy and stroke. The increasing of the body mass index (BMI) and plasma lipid profile could induce the change of blood pressure and atherosclerotic progression rapidly because of the alteration of glucose and lipid metabolism, fibrinotic activity, coagulation factor, blood viscosity and other metabolic changes. In Indonesia study about relationship between BMI and plasma lipid profile with diabetic neuropathy and stroke was limited.
Method: This was a cross sectional study of 49 subjects from stroke with control case and 53 subjects from diabetic neuropathy with control case. That was done at the H. Adam Malik General Hospital. It consisted of the stroke and diabetic neuropathy patients that were diagnosed from neurological examination and diagnostic procedures. Head computed tomography (CT) scan was done to diagnose stroke and the Diabetic Neuropathy Symptom (DNS) and Diabetic Neuropathy Examination (DNE) were done to diagnose diabetic neuropathy Body weight and height were measured to count the BMI and the blood vein was collected after 12 hours fasting for lipid profile examination.
Results: Of 106 patients, 53 diabetic neuropathy cases and 53 subject with no diabetic neuropathy, it was concluded that the possibility for diabetic neuropathy event was higher in underweight and obesity (p>0.05), Subjects with total cholesterol, triglyceride, LDL and HDL higher than normal have bigger possibility in diabetic neuropathy event (p>0.05). And also from 98 subjects, 49 stroke case and 49 subjects with no stroke, was concluded that the possibility for stroke event increased with the increasing of BMI, it was 3,67 times in obesity (p=0,046). The possibility for stroke events also increased with the decreasing of HDL and LDL plasma level (p>0.05).
Conclusion: In diabetic patients, diabetic neuropathy risk events were found higher in underweight, obesity patients and higher lipid plasma level (p>0.05). Stroke risk events were found higher in obesity patients and in patients with higher total cholesterol and triglyceride level and also in patients with low LDL and HDL plasma level. | en_US |

neuropati diabetikum dan stroke. lndeks massa tibuh (lMT) dan kadar lipid
plasma yang meningkat dapat menginduksi teriadinya perubahan, seperti
meningkatnya tekanan darah dan progesifitas aterosklerosis yang lebih cepat
akibat adanya perubahan metabolisme glukosa dan lipid, aktivitas fibrinolitik,
faktor pembekuan, viskositas darah dan perubahan metabolik lainnya. Da
lndonesia penelltian mengenai hubungan IMT dan kadar lipid plasma dengan
kejadian neuropati diabetikum dan stroke masih cukup terbatas.
Metode : Penelitian ini merupakan studi kasus potong lintang dengan 49 subyek
stroke dan kontrol serta 53 subyek neuropati diabetikum dan kontrol yang
dilakukan di Rumah Sakit Umum Haji Adam Malik. Subyek merupakan pasien
yang didiagnosis dengan stroke dan neuropati diabetikum berdasarkan
pemeriksaan neurologis dan pemeriksaan penunjang. Computed tomography
(CT) scan dilakukan untuk menegakkan stroke dan kuisioner Neuropathy
Diabetic Symptom (NDS) dan Neuropaw Diabetic Examination (NDE) dilakukan
untuk menegakkan neuropati diabetikum. Berat badan dan tinggi badan diukur
untuk menghitung IMT dan darah vena untuk pemeriksaan lipid plasma diambil
setelah dilakukan puasa selama 12 jam.
Hasil: Pada penelitian ini dari 106 subyek penelitian yang terdiri dari 53 kasus
neuropati diabetikum dan 53 subyek bukan neuropati diabetikum didapatkan
bahwa besar risiko paparan terjadinya neuropati diabetikum lebih tinggi pada
subyek undenve ight dan obesilas (p>0,05). Subyek dengan kadar kolesterol
total, trigtiserida, LDL dan HDL plasma yang lebih tinggi juga memitiki besar risiko
paparan lebih besar untuk terjadi neuropati diabetikum (p>0,05). Dan dari 98
subyek penelitian yang terdiri dari 49 kasus stroke dan 49 subyek bukan stroke
didapatkan bahwa besar risiko paparan terjadinya stroke meningkat seiring
dengan meningkatnya indeks massa tubuh (lMT), yaitu obesitas mempunyai
kemungkinan 3,67 kali (p=0,046). Besar risiko paparan terjadinya stroke juga
meningkat seiring dengan meningkatnya kadar koleslerol total dan trigliserida
serta menurunnya kadar LDL dan HDL plasma (p>0,05).
Kesimpulan : Risiko kejadian neuropati diabetikum pada penderita diabetes
melitus dijumpai lebih besar pada underweight dan obesitas serta kadar lipid
plasma yang tinggi (p>0,05). Dan risiko kejadian stroke lebih tinggi dUumpai pada
obesitas (p<O,05) dan pada subyek dengan kadar kolesteroa total dan trigliserida
yang tinggi serta kadar LDL dan HDL plasma yang rendah. | en_US |
