    Pengaruh Latihan Ketahanan Ekstremitas Bawah terhadap Kadar Laktat Serum dan Ambilan Oksigen Maksimal (VO2max) pada Penyakit Paru Obstruktif Kronik

    Abstract
    Background: Patients with COPD exhibit muscle atrophy and dysfunction which will accelerate lactic acid accumulation and decrease maximal oxygen uptake (VO2max) in tissues. This will in turn lead into early fatigue and decondition state. Considered as the best predictor of cardiorespiratory endurance and survival, VO2max can be attenuated by aerobic training' which hopefully will decrease lactate level. This study aimed to assess the effects of lower-limb endurance training on blood lactate level and VO2max in COPD patients Methods: An experimental study was conducted on 20 COPD patients in group C and D. Patients were prescribed lover-limb exercise by stationary cycling with individualized dose, twice a week for 4 weeks, ranging from 5-20 minutes/session- Blood lactate level was measured by spectrophotometric assay VO2max measurement was calculated pre-and post-intervention using Nury's Formula@ specifically designed for lndonesian (r=0,68; p<0.005), converting the 6-minute walking distance. Statistical analysis was performed by Wilcoxon test- Results: There was significant increase of VO2max value,(p<0-001) of all COPD subjects from 25.8+6.5 (baseline) to 28.3+6.9 mlkg -1min-1. The increase in group C patients was slightly higher than group D although statistically not significant (3,36 vs 2.03 mlkg-1 min -1; p-0.36). The VO2max increase was observed in all patients regardlless their severity of obstruction (p:0.44 However, in contrast to VO2max, blood lactate level increased slightly from 1.65+0.82 (baseline) to 1.86+1.08 mmol/L, although the increase was not statistically significant. There was no exacerbation occurred during rehabilitation program. Conclusion: Four weeks of lower-limb endurance training effectively increased VO2max of COPD patients, but did not affect blood lactate level significantly.
