    Abstract
    Background: Ovarian morphology findings on ultrasound such as antral follicle number, antral follicle size, ovarian volume, stromal volume, S/A ratio, pulsatility index (PI) and peak systolic velocity (PS) are important parameters in the diagnosis of PCOS. Polycystic ovary syndrome is divided into classical and nonclassical phenotypes. Objective: This study aimed to assess the morphological characteristics of PCO on ultrasound and its correlation to the PCOS phenotype. Methods: This research is an observational analytic study with a cross sectional design conducted at the Department of Obstetrics and Gynecology at the Adam Malik Hospital in Medan and at the USU Medical Faculty Network Hospital from May 2022 until August 2022. The collection of samples was carried out according to the inclusion and exclusion criteria. Samples were obtained as many as 20 people on the classic phenotype and 30 people on the non-classical phenotype. The data were then collected and tested using the chi square test or other alternatives. Results: Significant differences were found in the number of antral follicles (p=0,034), stromal volume (p=0,040), follicle distribution (p=0,037), and pulsatility index (p=0,048) between classical and non-classical phenotypes. There was a significant difference between classical and non-classical phenotypes with ratio of LH:FSH (p=0,040), testosterone (p=0,027), free testosteron index (p=0,001), sex hormone binding globuline (p=0,002), fasting insulin (p=0,001) and HOMA-IR (p=0,020). Body mass index (BMI) classical phenotype had a significant relationship with the number of antral follicles (p=0,036) and pulsatility index (p=0,043), while the non-classical phenotype BMI had a significant relationship with stromal volume (p=0,041). Conclusion: Differences in antral follicle count, stromal volume, and pulsatility index were found between classic phenotype and nonclassical phenotype.
