Background: In acute stroke has been studied many factors and is associated with increased morbidity and mortality. Thyroid hormones are known to have physiological effects on hemostatic and vascular systems including modulation of coagulation proteins. Changes in thyroid hormone levels that may occur in patients with acute stroke can affect the outcome Purpose: to evaluate association of thyroid hormones with lesion volume and the Modified Rankin Scale (mRS) of the 14th day post-acute stroke Methods: This cross-sectional study conducted in patients with acute stroke who were admitted to the ward Neurology RSUP. H. Adam Malik Medan. The selection of the study sample was determined by the method of non- probability sampling types consecutive sampling Results: It was found 58 acute stroke patients consisted of 31 (53.4%) ischemic stroke and 27 (46.6%) for hemorrhagic stroke. In the most category of ischemic stroke were found low levels of T3 were 19 subjects (32. 7%), normal T 4 category were 19 subjects (32. 7%) and normal TSH categories were 28 subjects (48.3%). In the most category of hemorrhagic stroke was found T3 levels low number of 20 subjects (34.5%), normal T4 category were15 subjects (32. 7%) and normal TSH category were 19 subjects (32. 7%). The Kolmogorov-Smimov test found no significant association between the levels of T3, T4 and TSH with lesion volume (P> 0.05). T3 levels were found only significantly associated with mRS score day 14 (P = 0.03). By using Somers'd correlation test found a negative correlation between levels of T3 with14th day mRS score (r = -0.547, P = 0.00) Conclusion: There is a significant relationship between the levels of T3 with 14th day mRS score in acute stroke with intermediate negative correlation in both.en_US
