    Abstract
Background: Changes in oxygen from the brain due to the effect of inhalation anesthetics on cerebral blood flow, where if brain blood flow decreases it can reduce brain oxygenation. This study aimed to analyze differences in cerebral oxygen levels in the use of Sevoflurane compared to Isoflurane as an inhalation anesthetic. Methods: This study was a double-blind randomized clinical trial study. This research was conducted at Haji Adam Malik General Hospital Medan. This study was conducted in August 2022. The sample of the study was laparotomy surgery patients at Haji Adam Malik General Hospital Medan who met the research criteria. The number of subjects who met the inclusion criteria were 26 patients with 13 groups receiving Sevoflurane and 13 groups receiving Isoflurane. The data to be collected was analyzed using the SPSS software program. Results: There are 26 patients with 13 patients in the Sevoflurane group and 13 in the Isoflurane group. Furthermore, both groups were randomized to obtain a balanced group of 13 patients in each group. In this study, the value of cerebral rSO2 saturation in the Sevoflurane group before induction compared to 5 minutes after induction and after surgery did not show a statistically significant difference, while the cerebral rSO2 saturation value in the Isoflurane group had a significant change in rSO2 value (p<0.05). before induction compared to 5 minutes after induction and after surgery. In this study also obtained a comparison of the rSO2 value of the Sevoflurane and Isoflurane groups where the rSO2 value has a significant difference at the time of examination of the rSO2 value 5 minutes after induction (T1) and after surgery (T2).. Conclusion: The role of Sevoflurane as an inhaled drug in Laparotomy surgery is better in maintaining cerebral oxygenation compared to the rSO2 value in NIRS which did not experience a significant decrease for surgery.
