    Hubungan Fragmentasi QRS dengan Severitas Penderita Gagal Jantung selama Rawatan di Rumah Sakit

    Abstract
    Background: Fragmented QRS (FQRS) is a convenient marker of myocardial scar evaluated by l2-lead electrocardiogram (ECG) recording. FQRS is defined as additional spikes within the QRS complex. FQRS was recently correlated with various outcomes in ischemic and non-ischemic heart disease. We hypothesized that the presence of FQRS would be associated with worse NYHA functional class and lower ejection fraction (EF) in heart failure Gm) patients of any cause. Methods'. Patients hospitalized for IIF due to ischemic or nonischemic were retrospectively analyzed. ECGs were read twice by a reader blinded to all data. Results'. Patients were classifed as FQRS group (n:50; mean age, 55.4+10.1 years) and non-FQRS group (n:40; mean age, 54.1114.6 years). FQRS on ECG was significantly correlated with higher number of leads with FQRS, FQRS involving >1 ECG areas, lower EF (<40%) and higher left ventricular end systolic dimension (LVESD) (47.1+11.7, mm) (p<0.001). There was significant difference in EF between patients with FQRS and non-FQRS group (p <0.001). Conclusion The occurrence of FQRS is beneficial to identify the patients with left ventricular contraction dysfunction. Presence of FQRS is associated with lower EF (<+OYo) in tIF patients. This was mainly seen in patients with FQRS involving 26 leads and >l ECG areas
     
    Latar Belakang'. Fragmentasi QRS (FQRS) pada elektrokardiogram (EKG) berperan dalam memprediksi adanya jaringan parut pada miokardial. FQRS pada EKG ditandai dengan adanya spikes tambahan pada kompleks QRS. FQRS berhubungan dengan beberapa keluaran yang buruk pada penderita gagal jantung akibat iskemik maupun non-iskemik. Salah satunya adalah kelas fungsional NYHA yang buruk dan fraksi ejeksi yang rendah. Methoda. Dilakukan pencatatan dan analisis data pasien gagal jantung akibat iskemik atau noniskemik yang dirawat di rumah sakit secara restrospektif. EKG akan dibaca oleh sebanyak dua kali oleh pengamat yang tidak mengetahui data pasien. Hasil. Penderita dibagi menjadi kelompok FQRS (n:50; rcrata usia, 55.4+10.1 tahun) dan non-FQRS (n:40; rerata usia, 54.1+14 6 tahun). FQRS berhubungan signifikan secara statistik dengan jumlah sadapan dengan FQRS, Zl area yang terlibat dengan FQRS, fraksi ejeksi rendah (<40oA), dan nilai left ventricular end systolic dimension (LVESD) diatas normal (47.1+11.7, mm) (p<0.001). Kesimpulanz FQRS berperan dalam identifikasi penderita dengan disfungsi kontraksi ventrikel kiri. Pada penderita gagaljantung, FQRS berhubungan dengan fraksi ejeksi rendah (<40oh), terutama dijumpai pada penderita dengan >6 sadapan dan >1 area yang terlibat dengan FQRS pada EKG
