    Perbandingan Pemberian Dexmedetomidine dan Midazolam terhadap Perubahan Kadar Interleukin-6 Paska Kraniotomi yang Menggunakan Ventilator di Unit Perawatan Intensif

    Abstract
    Background: The hurnan immune systern is complex, consisting of both inflammatory and immune cell reqponses. Anaesthesia and mayor surgery cause release of pro- inflammatory cytokines, especially interleukin-6 (IL-6) in excessive amounts which may cause postoperative complications, immune cell suppression, end- organ r5ury, multiple organ failure, and death. The aim of this study was to compare the efflect of bolus dexmedetomidin loading dose I mcg/kg followed by continuous dose 0,4 mcg&g/hour compared with bolus midazolam loading dose of 0,3 mg/kg followed by continuous dose 0,3 mg&glhour on the changes of IL-6 concentration on post-craniotomy patients who were admified in intensive care unit. Methods: Double-blinded, randomized clinical fiial on 38 patients admiued for craniotomy under general anaesthesia and had fulfilled inclusion and exclusion criterias, which later divided into 2 groups. 19 patiens recieved bolus dexmedetomidin loading dose I mcg/kg followed by continuous dose 0,4 mcg,/kg/hour and oflrer 19 recieved bolus midazolam loading dose 0,2 mgkg followed by continuous dose 0,3 mg/kg/hour. Hemodynamic was measured for both groups before sedation,6 hour, 12 hour and24 hour after sedation. Besides that, interleukin-6 concentration was measured before sedation and24 hours after sedation. The changes ofhemodynamic and IL-6 concentration were collected and analyzed by T-independqt test for normal distribution data and Mann-Whitrey test for non-normal distribution data. Results: There was a significantly suppressed concentration of interleukin-6 in dexmedetomidin group compared with midazolam group (p<0,001). Conclusion: There was a significantly suppressed concenfation of interleukin{ in patient post craniotomy and 24 hour post craniotomy administered with dexmedetomidin goup compared to midazolam group
     
    Latar Belakang dan Tujuan: Sistem kekebalan tubull manusia bersifat kompleks, terdiri dmi respon terhadap inflamasi dan respon imtur sel. Tindakan anestesi dan pembdahan menyebabkan pelepasan sitokin-sitokin pro-inflamasi, terutama interleukin-6 (IL-6) dalam jumlah yang berlebihan sehingga dapat menyebabkan komplikasi pascabedah, penekanan sistem imun, endargan injury, kegagalan organ multipel dan kematian. Penelitian ini bertujuan untuk mengetahui perbandingan efek bolasdexmedetomidin loading dosis I mcg/kg dilanjutkan dengan dosis 0,4 mcg/kgljam dan bolus midazolam loading dosis 0,2 mg/kg/jam dilanjutkan dengan dosis 0,3 mdkgljffi terhadap perubahan konsentrasi IL-6 pada pasien pasca kraniotomiyang dirawat di unit perawatan intensif. Metode: Uji klinis acak tersamar ganda pada 38 pasien yang menjalani kraniotomi dengan pembiusan umum dan telah memenuhi kriteria inklusi dan eksklusi yang terbagi menjadi dua kelompok. 19 pasien mendapat bolus de>anedetomidin loading dosis 1 mcg/kg dilanjutkan dengan dosis 0g mcg,rkg/jam dan 19 pasien lainnya mendapat bolus midazolam loading dosis 0,2 mg&g dilanjutkan dengan dosis 0,3 mg/kg/jam. Kedua kelompok tersebut dilakukan pemeriksaan hemodinamik saat paska kraniotomi, 6 jam, 12 jarn dan24jam paska kraniotomi. Selain itu, kedua kelompok juga diperiksa kadar IL-6 paska kraniotomi dan 24 jam paska kraniotomi. Data yang dikumpulkan berupa perubahan hemodinamik dan kadar IL-6, kemudian dianalisis menggunakan uji T- independent pada data berdistribusi normal dan uji Mann-Whitney pada data distribusi tidak normal. Hasil: Terdapat penrmman kadar interleukin-6 yang signifikan pada kelompok yang diberikan dexmedetomidin dibandingkan dengan kelompok yang diberikan midazolam (p<0,001). Kesimpulan: Terdapat penuunan kadar IL-6 pada pasien paska kraniotomi dan 24 jam paska kraniotomi terhadap kelompok yang diberikan dexmedetomidin dibandingkan dengan kelompok yang diberikan midazolam
