    Pengaruh Terapi Kortikosteroid Intranasal pada Polip Hidung Dinilai dari Perubahan Ekspresi Tumor Necrosis Factor Alpha (TNF-a)

    Abstract
    Background: Nasat polyp is chronic inflammation of nasal mucosa, which is known as edematous mass, hang down from the inflammation mucosa. Tumor Necrosis Factor (TNF-u) is one of the pro-inflammatory cytokine, which plays an important role in pathogenesis of nasal polyp by increasing secretion of VCAM-1 and RANTES in nasal fibroblast which regulate eosinophils infiltration. Fluticasone furoate (FF) is the most potent inhibitor in NF-kB activation and TNF release, which NF-kB pathway is known as a key factor in synthesis of pro-inflammatory cytokine and wide inflammation process. Besides, FF also inhibits other inflammatory mediator, such as histamine, eicosanoids, and leukotriene, and reduce protein secretion and inflammatory gene expression in nasal polyp fibroblast, which is influenced in reduction of nasal polyp size. Purpose: To determine the infleffectnce of Fluticasone Furoate nasal spray to TNF-o expression in nasal polyp. Methods: This is an analytic study with experimental design perform on 16 cases of nasal polyp, obtained by non probability consecutive sampling. lmmunohistochemistry is perform to assess TNF-o expression in nasal polyp before and after treatment with Fluticasone Furoate nasal spray for 4 weeks. Result:From 16 cases, most of TNF-o expression in nasal polyp before treatment with Fluticasone Furoate is positive (overexpression) (87,5%), whereas most of TNF-o expression in nasal polyp is negative afier treatment (81,3%). The analytic data shows a significant decrease in TNF- o expression after FF treatment (p=0,001). Decrease of nasal polyp stadium shows a significant result (p=0,001) but there is no significant relationship between the decrease of nasal polyp stadium with decrease of TNF-q expression after FF treatment. Wconclusion: Fluticasone Furoate proven significantly decrease TNF-o expression in nasal polyp and nasal polyp stadium
     
    Latar belakang: Polip hidung ialah penyakit inflamasi kronik dari mukosa hidung yang ditandai dengan massa edematosa yang bertangkai. Tumor Necrosis Factor (TNF-o) merupakan salah satu sitokin pro-inflamasi yang berperan penting dalam patogenesis polip hidung dengan cara meningkatkan sekresi dari VCAM-1 dan RANTES pada fibroblas hidung yang meregulasi infiltrasi eosinofil. Fluticasone furoate (FF) merupakan inhibitor paling poten dari aktivasi NF-kB dan lepasnya TNF, dimana NF- kB pathway dikenal sebagai faktor kunci dalam sintesis sejumlah besar sitokin pro-inflamasi dalam proses inflamasi yang luas. Selain itu FF juga menghambat mediator inflamasi lainnya seperti histamin, eicosanoids dan leukotrien serta mengurangi sekresi protein dan ekspresi gen inflamasi pada fibroblas polip hidung yang berpengaruh pada berkurangnya ukuran polip hidung. Tujuan : Mengetahui pengaruh Fluticasone furoate semprot hidung terhadap ekspresi TNF-o pada polip hidung. Metode: Penelitian ini bersifat analitik dengan desain studi kuasi eksperimental yang dilakukan terhadap 16 kasus polip hidung yang diperoleh dengan non probability consecutive sampling. Pemeriksaan imunohistokimia dilakukan untuk menilai ekspresi TNF-q pada polip hidung sebelum dan sesudah diterapi dengan Fluticasone furoate semprot hidung selama 4 minggu. Hasil: Dari 16 sampel, ekspresi TNF-o pada polip hidung sebelum diterapi dengan Fluticasone furoate terbanyak dijumpai positif (overexpression) (87,5o/o) sedangkan setelah diterapi terbanyak dijumpai negatif (81,3o/o). Analisa data menunjukkan penurunan ekpresi TNF-o yang bermakna sesudah diterapi (p=0,001). Dijumpai penurunan stadium polip hidung yang bermakna sesudah terapi dengan FF (p=6,601), akan tetapi analisa data tidak dijumpai hubungan yang bermakna antara penurunan stadium polip hidung dengan penurunan ekspresi TNF-o pada polip hidung setelah diterapi dengan FF (p=9,136;. Kesimpulan : Fluticasone furoate terbukti menurunkan ekspresi TNF-o pada polip hidung dan mengurangi stadium polip secara signifikan
