Background Stroke is a global health problem whose incidence is increasing every year around the world. So it is necessary to identify the early complications of stroke that occur during treatment that affect stroke mortality and morbidity. Pneumonia is a complication that is often experienced by stroke sufferers. Until now there are several scoring systems or certain laboratory parameters that can be used to predict the incidence of pneumonia in acute ischemic stroke patients Objective This study was conducted to compare the accuracy of the AIS-AP score, ISAN score, A2DS2 score, PANTHERIS score, Vitamin D level and serum ferritin level on the incidence of pneumonia in acute ischemic stroke patients. Method This study used a prospective cohort method with an analytical observative approach in patients with acute ischemic stroke. The subjects of the study were 33 acute ischemic stroke patients who were assessed for AIS-AP score, ISAN score, A2DS2 score for PANTHERIS score, Vitamin D Levels and Serum Feritin Levels on the first day of admission and assessed whether they had pneumonia. Result The AIS-APS results obtained a sensitivity score of 90.4%, specificity of 75%, accuracy of 84% and AUC of 85.3%. ISAN score sensitivity 85%, specificity 66.7%, accuracy 81% and AUC 82.3%. A2DS2 score sensitivity 85.7%, specificity 66%, accuracy 78% and 89.5% AUC. PANTHERIS score sensitivity 85%, specificity 75%, accuracy 81% and AUC 83%. Serum Feritin sensitivity 80%, specificity 83%, accuracy 81% and AUC 83.9%. Vitamin D Serum sensitivity 71%, specificity 83%, accuracy 75% and AUC 81%. Conclusion The use of scores and laboratory parameters has good accuracy and ability to predict the incidence of pneumonia in acute ischemic stroke, where the AIS-AP score has a highest level of accuray compared to ISAN Score, A2DS2 Score, PANTHERIS score, serum feritin and vitamin D levels.en_US
