Disease surveillance activity is conducted to carry out the supervision continuously on the distribution and the inclination of illness incidence through systematic collecting, consolidation, evaluation on the reports of illness and death, and other relevant data which can be analyzed by using systematic approach: input, process, and output. The objective of the research was to evaluate the implementation of disease surveillance activity for the evacuees in the emergency response to the disaster of Mount Sinabung eruption in Karo District in 2014, based on the policy of Disease Surveillance of the Department of Health of the Republic of Indonesia. The research used qualitative method, and the informants were the surveillance personnel of the District Health Office and the surveillance personnel at Primary Health Center. The instruments in the research were the guidance from interviews and documentation study and analyzed by using triangulation method. The result of the research showed that the implementation of disease surveillance activity was not performed well. In its implementation, it indicated that there was weaknesses in input in the indicator of human resources and of facilities, the process of interpretation indicator and data analysis and data dissemination, and the output of the indicator of making the profile of epidemiology. It is recommended that the management of the District Health Office transfers knowledge through socialization and training about the activity of disease surveillance in disaster and crisis for the personnel so that they will yield complete and correct data to be used as a reference in making the policy.
