    Abstract
    Background: The Maternal Mortality Rate (MMR) is an indicator of the success of maternal health efforts. Maternal and Child Health Efforts (MCH) is one of the priority programs at the Community Health Center which is expected to be able to reduce the maternal mortality rate (MMR), neonatal mortality rate (NMR), infant mortality rate (IMR) and under-five mortality rate (UMR). After adaptation to the profession and related programs, it is agreed that in Indonesia it is 6 visits where two visits are in the first trimester, one visit in the second trimester and three visits in the third trimester and two visits to the doctor. Incomplete MCH handbook filling will have an impact on not monitoring the health of pregnant women and their fetuses. The purpose of this study was to assess the quality of ANC according to the MCH handbook at the Community Health Centers in Medan City. Research Method: This research is an observational analytic study with a cross- sectional design conducted by 21 Community Health Centers in 135 pregnant women undergoing ANC and 37 midwives conducting ANC at the health center from October to November 2022. Pregnant women were given a questionnaire and viewed the completeness of the MCH and midwife books only given a questionnaire. The data were analyzed descriptively to see the midwives' characteristics, a normality test was carried out with the Kolmogorov Smirnov to assess the relationship between the characteristics of pregnant women and the quality of ANC and bivariate analysis was also carried out. Results: The most influential characteristic of pregnant women was education with 66 pregnant women were graduated from high school (48.9%) with incomplete MCH books, statistically significant (p = 0.003) and frequency of ANC visits <6 times as many as 134 people (99.2 %) with incomplete MCH handbooks, statistically significant (p = 0.007). The average ANC visit to the public health center was 2.32 ± 1.514 visits. Conclusion: From this study it was found that the quality of ANC at the Community Health Centers in Medan City is still far from good because the mean number of visits is still below 6 times and the majority of MCH books are incompletely filled out.
