The standard of family and community health, among other things, is determined by the standard of mother and child health as a critical and strategic group of community. Therefore, it is a need to minimize the maternal and infant mortality rate which is an indicator in evaluating the standard of community health. Antenatal care is intended to keep the mother and the baby she is carrying being healthy to the end of her pregnancy. For this purpose, a mother should have an antenatal care examination at least 4 times during her pregnancy - one visit in the first trimester, one in the second trimester, and two visits in the third trimester. On the first visit (K1), the pregnant mother has her pregnancy examined by the health worker to get a standard antenatal care examination. The fourth visit (K4) is the fourth contact or more the pregnant mother makes to have an antenatal care examination. In Indonesia, antenatal care is part of Mother and Child Care (KIA) program. The purpose of this descriptive study is describe the recording and reporting system of Mother and Child Health Service program in the District of Langkat which is, in general, good enough, but does the result show the condition of real life situation? Maternal mortality rate in this district is lower than that in Indonesia. Are the data obtained by the researcher from the Puskesmas (Community Health Centers) in the District of Langkat valid and in accordance with current condition? The result of this study shows that there are limitations in the recording and reporting system of Mother and Child Health program in the District of Langkat that the data obtained do not match the actual condition on the field caused by the poor control on the field and lack of awareness of the health workers themselves. Based on the problem above, the Puskesmas staff/workers need to be continuously monitored in order to improve the recording and reporting system of the Mother and Child Health program. The performance quality of human resources in the health sector needs to be improved. The staff needs to be controlled in implementing their task, especially in the reporting system, and their awareness on the importance of reporting system also needs to be improved.
