    Korelasi Antara Kadar Leptin Serum dengan Indeks Massa Tubuh Pasien Skin Tag

    Abstract
    Latar belakang : Skin tag merupakan tumor jinak jaringan konektif kulit, berbentuk sessile atau pedunkulasi berwarna kecoklatan atau sewarna dengan kulit dan sering timbul pada daerah fleksura. Skin tag dapat dijadikan sebagai penanda dini/prediktor adanya gangguan metabolisme lipid yang dipengaruhi oleh hormon leptin. Obesitas berhubungan dengan tingginya kadar leptin didalam tubuh dimana individu obesitas lebih sering mengalami skin tag dibandingkan individu dengan indeks massa tubuh normal. Tujuan: Mengetahui korelasi antara kadar leptin serum dengan indeks massa tubuh pasien skin tag. Subjek dan metode: Penelitian ini merupakan penelitian analitik observasional dengan desain potong lintang yang melibatkan 33 pasien skin tag dengan kategori indeks massa tubuh (IMT) normal, overweight dan obesitas. Diagnosis skin tag ditegakkan berdasarkan anamnesis dan pemeriksaan klinis, selanjutnya dilakukan pengukuran IMT dan pengambilan sampel darah dari subjek penelitian untuk pemeriksaan kadar leptin serum. Hasil: Pada penelitian ini mendapatkan rerata kadar leptin serum pasien skin tag paling tinggi pada kelompok usia 40-49 tahun (36,21 ± 10,03 ng/ml). Rerata kadar leptin serum pasien skin tag perempuan (36,98 ± 17,67 ng/ml) lebih tinggi dibandingkan laki-laki (28,32 ± 12,04 ng/ml). Rerata kadar leptin serum pasien skin tag dengan riwayat keluarga (34,69 ± 12,77 ng/ml) lebih tinggi dibandingkan dengan tanpa riwayat keluarga (20,30 ± 8,63 ng/ml). Rerata indeks massa tubuh pasien skin tag paling tinggi pada usia 40-49 tahun (25,62 ± 2,63 kg/m2). Rerata indeks massa tubuh pasien skin tag laki-laki (24,67 ± 3,60 kg/m2) lebih tinggi dibandingkan dengan perempuan (24,43 ± 2,12 kg/m2). Rerata indeks massa tubuh pasien skin tag dengan riwayat keluarga (25,01 ± 2,81 kg/m2) lebih tinggi dibandingkan dengan tanpa riwayat keluarga (24,09 ± 2,29 kg/m2). Rerata kadar leptin serum pada pasien skin tag dengan indeks massa tubuh obesitas (42,89 ± 11,39 ng/ml) lebih tinggi dibandingkan dengan pasien skin tag dengan indeks massa tubuh overweight dan normal yaitu masing-masing sebesar 26,38 ± 9,71 dan 20,41 ± 6,89 ng/ml. Pada penelitian ini didapatkan korelasi positif kuat (r=0,716) antara kadar leptin serum dengan indeks massa tubuh pasien skin tag dengan p<0,05. Kesimpulan: Terdapat korelasi positif kuat antara kadar leptin serum dengan indeks massa tubuh pasien skin tag.
     
    Background: Skin tag is a benign tumor of connective tissue in the skin, sessile or pedunculated, skin-like to brownish colored and often arises in the flexure area. Skin tag as an early marker/predictor of impaired lipid metabolism that are affected by hormon leptin. Obesity is associated with high levels of leptin where skin tag commonly develop in individuals with obesity than individuals with normal body mass index. Objective: To determine correlation between serum leptin levels with body mass index in skin tag patients. Subjects and Method: This study is an observational analytic study with a cross-sectional design involving 33 skin tag patients with body mass index of normal, overweight and obesity categories. Diagnosis of skin tagwas made based on history and clinical examination, furthermore subjects performed measurement of body mass index and blood sample examination leptin level. Results: In this study, we found the mean serum leptin levels of skin tag patients were highest on aged group 40-49 years (36.21 ± 10.03 ng/ml).The mean serum leptin level of female skin tag patients (36.98 ± 17.67 ng/ml) was higher compared to male (28.32 ± 12.04 ng/ml). The mean serum leptin level of skin tag patients with a family history (34.69 ± 12.77 ng/ml) was higher compared to patients without family history (20.30 ± 8.63 ng/ml). There was a highest mean body mass index in skin tag patients on age group 40-49 years (25.62 ± 2.63 kg/m2). The mean body mass index of male skin tag patients (24.67 ± 3.60 kg/m2) was higher compared to women (24.43 ± 2.12 kg/m2). The mean body mass index was higher in skin tag patients with a family history (25.01 ± 2.81 kg/m2) compared to patients without a family history (24.09 ± 2.29 kg/m2). The mean serum leptin level in skin tag patients with body mass index of obesity was higher (42.89 ± 11.39 ng/ml) compared with skin tag patients with body mass index of overweight and normal, which was 26.38 ± 9.71 and 20.41 ± 6.89 ng/ml. There is a highly positive correlation between serum leptin levels (r=0,716) with body mass index of skin tag patients with p<0,05. Conclusion: There is a highly positive correlation between serum leptin levels with body mass index of skin tag patients.
