Introduction: Patient diagnosed with STEMI especially those who are at high risk, may develop MACE in the future, even with optimal reperfusion and medical treatment. Outcomes Registry for Better Informed Treatment (ORBIT) score is a new scoring system used to assess bleeding risk in patient with AF receiving anticoagulant, but it’s role in predicting Major Adverse Cardiovascular Events (MACE) in STEMI cases is still very rare, especially in Indonesia. Objective : To determine the relationship between ORBIT score as a predictor of MACE and bleeding during admission in STEMI patients underwent PCI. Method: This study is an observational cohort analysis of ambiseffective in 120 samples diagnosed with STEMI receiving PCI as reperfusion therapy in RSUP H. Adam Malki.. All subjects who underwent PCI are counted for ORBIT score, then observed during hospitalization. The output that was monitored was MACE and bleeding incidents during hospitalization. The predictive value of ORBIT score was analyzed using ROC curve. Result: From 120 samples enrolled in this study, 34 patients (30%) had MACE. The ORBIT score was higher in the group that experienced MACE (P<0.001). ROC analysis results ORBIT score has a good ability to predict MACE (AUC: 0.852, 95% CI: 0.771-0.932 p<0.001). There was a statistically significant relationship with ORBIT score on each MACE which are death, cardiogenic shock, acute heart failure, and malignant arrhythmias with p value <0.05. There were 14 patients (11.6%) who had bleeding. ORBIT score has moderate predictive ability in predicting bleeding events (AUC 0.728 95% CI: 0.570-0.886 p<0.001) Conclusion: ORBIT score can be used as a predictor of MACE and bleeding during hospitalization in STEMI patients who underwent PCIen_US
