    Abstract
    Datu Beru Hospital is a public hospital in Takengon with its low coverage in IMD (Early Breastfeeding Initiation). There are 47,3 percent of newborn babies through normal childbirth got IMD, and only 14,5 percent of newborn babies through operation got IMD in 2020. Even though the IMD has surpassed the target, its implementation was still not in accordance with the expected procedure. The objective of the research is to analyze the implementation of IMD in this hospital. The research uses qualitative methode with phenomenological approach. It is conducted from December, 2020 to August, 2022. The data are gathered by conducting in depth-interview from February to April, 2022. The result of the research shows that the implementation of IMD has not run well due to some aspects- 1) The measurement and the purpose of the policy, the management and health care providers do not have the same viewpoint about the standard and the objective of IMD, 2) Communication: both health care providers and patients do not get clear information about IMD, 3) Resources: the hospital management do not use resources maximally, 4) Disposition: lack of commitment and role of health care providers in the implemention of IMD, 5) Characteristics of executing agent: there are no sanctions from hospital management concerning the standardized implementation of IMD, 6) Social condition: local people still believe in stigma of breastfeeding containing coloctrums, and 7) Environmental condition: cold weather has caused the process of IMD not to take a long time. It is recommended that the hospital management to streamline the implementation of the IMD by doing supervision, and evaluation sustainably about IMD program, punishing their health care providers who do not comply with the IMD procedure, make special policy about prohibition to give powdered milk to all newborn babies, and provide IMD education for all the pregnant woman since they have their pregnancy examined.
