Introduction: Non-ST Segment Elevation Acute Myocardial Infarction (NSTEMI) is a part of ACS (Acute Coronary Syndrome) with an NSTEMI incidence of 58% and a mortality during treatment of 2.8%. The SYNTAX score is a tool used to assess the complexity of coronary artery disease. Fibrinogen to albumin ratio (FAR) is a biomarker of inflammation and coagulation that has a linear correlation with the SYNTAX score which shows its potential in assessing the severity of coronary lesions. This study aims to determine the relationship of the fibrinogen to albumin ratio to predict the severity of coronary artery lesions in NSTEMI patients. Method: This study is an analytic descriptive study with a case control study design by assessing the relationship between the fibrinogen to albumin ratio to the severity of coronary lesions in NSTEMI patients at H. Adam Malik General Hospital Medan from February 2023 to June 2023. The value of the fibrinogen and albumin ratio was calculated before coronary angiography is performed, the SYNTAX score is calculated based on the results of coronary angiography. Bivariate test was conducted to assess the correlation between the two variables. Furthermore, ROC analysis was performed to assess the relationship between fibrinogen and albumin ratio as a predictor of coronary lesion severity. Result: Total subjects were 52 NSTEMI patients consisting of 26 (50%) patients with moderate-severe coronary lesions, 26 (50%) patients with non-severe coronary lesions. From the ROC analysis to assess the power of the fibrinogen to albumin ratio in predicting the severity of coronary lesions with an AUC value of 83.7% which is statistically significant has a strong accuracy (p <0.001) with a cut off point 109.55 with a sensitivity of 84% and a specificity of 84%. Conclusion: The fibrinogen to albumin ratio can predict the severity of coronary lesions based on the SYNTAX score in NSTEMI patients.en_US
