Introduction: Left ventricular ejection fraction (LVEF) was not included in
previous models for risk stratification in NSTEMI patients, but its reduction
portends a worse prognosis. Several studies have characterized
electrocardiographic (ECG) patterns correlated with left ventricular dysfunction in
this population. The PQRST score is a score that can predict reduced LVEF in
NSTEMI patients. This study aims to determine the relationship between PQRST
score and left ventricular systolic function in NSTEMI patients.
Methods: This is a cross-sectional study of 171 patients with NSTEMI at RSUP
Haji Adam Malik Medan. Data was collected from April 2023 until the sample was
fulfilled. The PQRST score was calculated to be classified into a high risk score
and a low risk score. Bivariate test was conducted to assess the relationship
between the two variables. Furthermore, an analysis with the ROC curve was
performed to assess the PQRST score in predicting a decrease in LVEF and
mortality during hospitalization.
Result: The number of patients with reduced LVEF was 93 (54.4%) patients. There
is a relationship between the PQRST score group and reduced LVEF (p<0.001).
The results of the ROC analysis showed that the PQRST score was a good predictor
to predict LVEF (p<0.001; AUC=0.95). The PQRST score could predicted inhospital
mortality, but not better than the GRACE score (p<0.001; AUC 0.78 and
p<0.001; AUC 0.80).
Conclusion: The PQRST score has a relationship with left ventricular systolic
function and could predict LVEF in NSTEMI patients at Haji Adam Malik General
Hospital Medan.
