    Association Between Serum Chloride Levels and Length of Stay in Patients with Acute Decompensated Heart Failure Upon Admission to Haji Adam Malik

    Abstract
    Background : Electrolyte disturbances are common in patients with acute decompensated heart failure. Many studies of serum sodium levels in acute heart failure have shown poor outcomes. Recent studies have suggested that serum chloride levels can be a predictor in patients with acute heart failure. In this study, correlation between blood serum chloride levels on admission and length of hospital stay was analyzed. Methods : This study is an ambispective cohort study. Sample was collected at RSUP HAM start from January 2021 until the number of samples was fulfilled. Correlation and binary logistic regression tests were performed to see the relationship and influence of serum chloride on admission with length of hospitalization. Results : A total sample of 140 patients with acute decompensated heart failure was collected. The median serum chloride level was found to be 102 (97 - 106) meq/L. Patients were divided into 3 groups based on blood chloride levels, < 98 meq/L, 98 - 106 meq/L, > 106 meq/L. From the correlation test results, there was a relationship between serum chloride levels at admission and length of treatment with a weak negative correlation (r: - 0.271; p = 0.001). This study also found a strong positive linear relationship between chloride levels and sodium levels at admission (r: 0.670; p < 0.001). After binary logistic regression test, it was found that hypochloremia and hyponatremia groups had an influence on the length of treatment > 7 days (OR 3.47; IK 1.40 - 8.63; p value 0.007). Conclusions : Serum chloride levels on admission may be a predictor of length of stay in hospital in patients with acute decompensated heart failure.
