    Abstract
    Introduction Early fluid resuscitation is essential to stabilize tissue hypoperfusion and crystalloid is the mainstay of choice for resuscitation in septic shock. The aim of this study to determine the effectiveness of isotonic compared to hypotonic in septic shock resuscitation. Method The experimental study with open label method, was enrolled the septic shock children aged 1 month - 18 years old on September to December 2022 at H. Adam Malik Hospital. The eligible samples divided into 2 groups, they received isotonic solution or hypotonic solution. The data were analyzed with SPSS Ver. 25 program (95% CI and a significance p <0.05). Results The total of 42 children were analysed. No significant difference in demographic characteristics between the two fluids. No significant difference between characteristic hemodynamic (stroke volume, cardiac index and systemic vascular resistance) and isotonic solution at initial or after 24 hours. Likewise with hypotonic solution, no significance characteristics hemodynamics at initial or after 24 hours. There were significance difference improvement of consciousness between isotonic and hypotonic in septic shock resuscitation with p value 0,009. There were significance improvement of blood pressure between isotonic and hypotonic in septic shock resuscitation with p value 0,011. Conclusion Isotonic solution more effective than hypotonic solution in improvement of consciousness and blood pressure in children with septic shock resuscitation.
