    Abstract
    Background: Atrophic acne scars occur because of skin damage during healing process. Transforming growth factor-b (TGF-b) levels which decreases during the acne healing process plays a role in the formation of atrophic acne scars. The peel of red dragon fruit (Hylocereus polyrhizus) has a high antioxidant content, can increase TGF-b levels thereby stimulating an increase in fibroblast cells and the formation of new blood vessels for collagen synthesis. Objective: To determine the effect of red dragon fruit peel extract capsules on TGF-β levels in atrophic acne scars. Subjects and Methods: This study was a Quasi experimental clinical trial with a pretest-posttest design involving 30 subjects aged ≥18 years with atrophic acne scars. Basic data were recorded, subject examinations based on qualitative scarring grading system by Goodman and Baron, and measurements of TGF-β levels before and after administration of red dragon fruit peel extract capsules 400 mg for 8 weeks. Data were analyzed using the Shapiro-Wilk test and continued with the Paired T test. Results: Atrophic acne scars occurs more frequently in the 26–35 years age group, 15 subjects (50%) with the most gender being women, 18 subject (60%). There was an increase in TGF-β levels in subjects after being given red dragon fruit peel extract capsules which was statistically significant (p=0,022). Administration of red dragon fruit (Hylocereus polyrhizus) peel extract capsules did not show a decrease in the degree of atrophic acne scars. Conclusion: Red dragon fruit (Hylocereus polyrhizus) peel extract capsules can increase TGF-β levels in atrophic acne scars.
