    Abstract
    Background: Anesthesia-related mortality can result from complications such as pulmonary aspiration, asphyxia, or anaphylaxis. Airway management is challenging, including predicting intubation difficulty. Skin-to-epiglottis distance (DSE) may be a predictor. However, studies have not analyzed ultrasound-assisted intubation difficulty using DSE measurements and scoring systems such as LEMON and Wilson in patients undergoing general anesthesia. Methods: This cross-sectional observational design study aimed to evaluate the correlation between DSE, LEMON score, and Wilson score in predicting intubation difficulty in patients who received general anesthesia at Hajj Adam Malik Hospital Medan from March to April 2024. Consecutive sampling was used to collect 76 samples that met the inclusion and exclusion criteria. Results: Based on the results of the Fisher Exact test analysis, it is known that DSE, Lemon, and Wilson scores have a significant relationship with successful intubation (p=0.003; 0.001; 0.001 respectively). DSE has the highest sensitivity (94.9%), WILSON has the highest specificity (58.8%), Wilson score has the highest NPP (88.1%), and DSE has the highest NPN (66.7%). Conclusion: DSE, LEMON score, and Wilson score have a significant relationship with intubation success, with DSE having the highest sensitivity and LEMON score having the highest specificity compared to other modalities.
