T. Sari Gunung Berastagi is a manufacturing industry that produces passion fruit syrup, starting with the procurement process of passion fruit as raw material which serves as the input for the syrup production management process. Based on market demand data for passion fruit syrup, there are periods when demand cannot be met due to consumer trends and the availability of passion fruit, influenced by seasons, prices, and quality. PT. Sari Gunung Berastagi frequently experiences conditions of stockouts and overstocks, as well as delays in the procurement of raw materials, with 4 instances of overstock and 6 instances of stockout during the year 2023. The issues arise from the mismatch between raw material inventory and market demand. This research aims to identify risk events and risk agents within the company's supply chain, and to determine handling strategies to reduce the likelihood of risk agents arising. The data collection method for this study involved the entire company's employee population, with the sample selected being company executives due to their relevant knowledge. Based on the identification results, 26 risk events and 26 risk agents were identified using the supply chain operation reference. Based on house of risk I calculations, 14 priority risk agents were obtained to carry out mitigation actions. Where the highest priority risk agent with the highest aggregate risk potential value was 9.51%, namely the Supplier sent raw materials that did not match the quantity and quality of raw materials. The results of House of Risk I are used as input for House of Risk II and produce 15 risk mitigation actions that can be proposed to the Company. Mitigation actions that are effective and easy for the Company to carry out based on effectiveness to difficult calculations are considering the selection of suppliers in different locations. This action can minimize shipping costs and delivery timesen_US
