    Abstract
    Background: Diabetes mellitus patients often have comorbid diseases that require them to receive multiple therapy, this situation carries a risk of causing drug-related problems (DRPs). Objective: The aim of this study is to determine whether there is an incidence of DRPs, as well as differences in the number of DRPs incidence based on the number of diagnoses, number of types of medication, and duration of hospital stay in inpatient diabetes patients at H. Adam Malik General Hospital Medan in 2023. Method: The study used a descriptive method with retrospective data collection sourced from medical records. Identification of DRPs is carried out based on DRPs categories according to Cipolle 2012. Results: There were 52 diabetes mellitus patients who met the criteria. A total of 38 patients experienced 88 cases of DRPs consisting of needing additional drug therapy (36,3%), drug interactions included in the too high dose category (36,3%), too high dose (13.6%), drug interactions included in the adverse drug reactions (4.5%), adverse drug reactions (3.4%), ineffective drugs (2.3%), interactions involving too low a dose (2.3%), and unnecessary drug therapy (1.1%). Kruskal-Wallis test results showed that there was a significant difference in the number of DRPs events based on the number of drugs and duration of stay, each of which had a value (p < 0.05), while there was no significant difference in the number of diagnoses (p > 0.05). Conclusion: There was an incident of DRPs at H. Adam Malik Hospital. There were differences in the number of DRPs occurrences based on the number of drugs and duration of stay, meanwhile, there were no differences DRPs occurrences based on the number of diagnoses.
