    Abstract
    Background: Polycystic Ovary Syndrome (PCOS) is a metabolic and reproductive disorder affecting women of reproductive age, characterized by hyperandrogenemia, ovarian dysfunction (chronic anovulation and infertility), and polycystic ovaries, and is associated with metabolic consequences, including insulin resistance, type 2 diabetes, and obesity. HOMA-IR is the gold standard method used to assess insulin resistance. Methods: This study employed a quasi-experimental pre- and post-test design to analyze the effect of lipase inhibitor therapy (orlistat) on HOMA-IR in PCOS patients. The study included 25 subjects, with a 20% dropout rate, resulting in a total of 30 subjects. Blood samples were collected from the cubital vein, followed by statistical analysis of the data. Results: The majority of PCOS women were aged >30 years (55,2%) with BMI category in overweight (34,5%) or obese (37,9%). Of the 29 patients, only 8 patients (27,6%) were diagnosed as lean PCOS, There was no significant difference in HOMA-IR values between before and after orlistat administration in subjects aged > 30 years and under < 30 years (p=0,416 and 0,266), The mean body weight before and after administration was not statistically significantly different (p = 0,641), Mean HOMA-IR in obese PCOS women before and after intervention conducted by bivariate analysis showed a significant difference in HOMA-IR levels based on BMI before intervention (p=0,049) and after intervention (p=0,01) orlistat therapy Conclusion: Administration of lipase inhibitors to PCOS patients has the effect of reducing HOMA-IR values in obese subjects, but has less effect on overweight and normoweight subjects (lean PCOS).
