Background : Melatonin is a hormon created by pineal gland to help initiate and
maintain a sleep state. Melatonin according to previous research has been found to
promote neuroplasticity and acted as an antioxidant that leads to reduced infarct
volume in ischemic stroke model rats.
Objectives : To observe changes in Melatonin serum level of acute ischemic stroke
patients after Melatonin 10 mg supplementation for 7 days and its relationship to
the improvement of sleep quality and clinical outcome
Methods : This research was conducted in quasi experimental manner with 40
consecutive eligible subjects obtained from two different hospitals in Medan. All
subjects received pre- and post-supplementation assesment of serum Melatonin,
PSQI score and NIHSS score. Statistical analysis thus performed to determine
demographic characteristics, comparison of serum Melatonin level before and after
supplementation, and its relationship with changes in PSQI score and NIHSS score.
Results : Majority of subjects were male (52,5 %), had Batak ethnicity (52,5%) and
median age of 58,5 years old. There was a difference in median Melatonin serum
levels of day 1 (75,55 pg/mL) compared to day 7 (84,46 pg/mL) although not
statistically significant (p = 0,307). There are no significant relationship between
delta value of serum Melatonin towards PSQI (p = 0,198 ; r=0,208) and between
delta value of serum Melatonin towards NIHSS (p = 0,890 ; r=-0,023) amongst
subjects. No serious adverse effects were reported.
Conclusion : Serum Melatonin did increased after 7 days supplementation of
Melatonin. No relationship was found between changes in Melatonin serum
towards sleep quality and ischemic stroke severity.
