Background: Skin aging is a process that takes place progressively, multifactorial, and complex. Mango (Mangifera indica L.) originates from South and Southeast Asia. Mangifera indica L. var. kelong is a mango variety cultivated in North Sumatra. Mango leaves contain polyphenols, namely mangiferin, which plays a role in improving the appearance of aging skin with antielastase, anticollagenase, antihyaluronidase, and antityrosinase effects. Objective: aimed to evaluate the effects of a 3% mango leaf extract cream (Mangifera indica L. var. kelong) on facial skin aging. Subjects and Methods: We conducted this pre-experimental clinical trial research with a pretest-posttest design on 40 subjects with aging facial skin. We confirmed the diagnosis through history and clinical evaluation using the dermoscopic photoaging scale (DPAS) and skin analyzer before and after administering 3% mango leaf extract cream at weeks 0, 2, 4, 6, and 8. During the study, we recorded side effects and assessed the level of satisfaction at the end of the research period. To determine the differences before and after treatment, we conducted the repeated ANOVA and Friedman test, considering a p<0.05 as significant. Results: There was a statistically significant reduction in the DPAS score in subjects with facial skin aging after 8 weeks of applying a 3% mango leaf extract cream. According to the skin analyzer, there was a significant reduction results for spot evaluation, roughness, and ultraviolet moisture, while wrinkles, ultraviolet damage, and ultraviolet pigmentation showed a decrease in mean results, although not significant. All subjects found the 3% mango leaf extract cream satisfactory and did not experience any side effects. Conclusion: The use of 3% mango leaf extract cream can improve the appearance of aging facial skin without side effects and good level of satisfaction.en_US
