    Abstract
Diabetes mellitus is a chronic disease that requires continuous treatment in order to maintain blood sugar levels and mitigate complications. Non-adherence to medication among diabetes mellitus patients can worsen the disease and increase the risk of complications. This Research adopts a quantitative approach with a cross-sectional design. The Research population consists of diabetes mellitus patients who regularly visit Puskesmas Namorambe throughout 2023, with a total 871 visits. The sample is taken using a total sampling method, involving an average of 73 patients per month. The Morisky Medication Adherence Scale (MMAS-8) serves to assess the adherence, while a glucometer measures blood sugar level. The Research finds that the majority of patients have low adherence levels (41 respondents) and random blood sugar levels 200 mg/dl (43 respondents). Statistical analysis using Spearman's Rho test demonstrates a significant correlation between medication adherence and blood sugar levels (p-value 0.005), indicating a moderately strong and inverse correlation. Enhancing education and support for diabetes mellitus patients is essential to improve medication adherence, achieve better blood sugar control, and reduce the risk of complications. Regular blood sugar monitoring and medication adherence at Puskesmas Namorambe should be improved through comprehensive and sustainable health programs.
