    Abstract
    Background: Non-communicable diseases have become a global challenge. The burden of these diseases continues to rise. Primary health centers serve as gatekeepers in the initial management, with pharmacists as one of the frontlines in the effort to achieve effective therapy. Objective: The aim of this study was to analyze the impact of pharmacist counseling on medication adherence, clinical outcomes, and Quality of Life (QoL) in outpatients at Puskesmas Simpang Mamplam. Methods: This is a quasi-experimental study with one group pretest posttest design, using a total sampling technique included 112 DM2 respondents, 150 Dyspepsia respondents, 133 Neuropathy respondents, 110 Arthritis respondents who are outpatients at Puskesmas Simpang from March to April 2024. Data collection carried out using validated questionnaires. The data was analyzed using IBM SPPS 21. Result: The results showed a significant effect of pharmacist counseling on DM2, Neuropathy, Dyspepsia, and Arthritis patients’ adherence (8,79±1,29; 8,71±1,17; 9,28±0,63; 8,38±0,8), clinical outcomes 130,00±27,77; 2,88±1,36; 0,92±1,203; 1,39±0,73), and QoL (88,19±0,47; 83,55±18,77; 93,97±6,04; 83,44±12,68). A significant correlation between adherence and outcome was found in each disease group (r for DM2, Neuropathy, Dyspepsia, Arthritis: -0.225; -0.222; -0.231; -0.598). The r result for correlation test between adherence and QoL of DM2, dyspepsia, neuropathy, and arthritis were 0,107; 0,263; 0,210; 0,621. Correlation between QoL and outcome generated r for DM2, Neuropathy, Dyspepsia, Arthritis: -0,035; 0,416; 0,030; -0,546). Conclusion: It concluded that pharmacist counseling has positive impact on adherence, QoL and clinical outcomes. Meanwhile, the relationships among variables vary and are influenced by factors according to the type of disease.
